鼻内镜手术辅助低温等离子射频消融术对鼻腔鼻窦内翻性乳头状瘤患者鼻功能、复发率的影响

Effect of Nasal Endoscopic Surgery Assisted by Cryogenic Plasma Radiofrequency Ablation Therapy On Nasal Function and Recurrence Rate In Patients With Nasal And Sinus Inverted Papilloma

目的:探讨鼻内镜手术辅助低温等离子射频消融术治疗对鼻腔鼻窦内翻性乳头状瘤(SNIP)患者鼻功能、复发率的影响。方法:选取2021年3月—2022年3月焦作煤业(集团)有限责任公司中央医院收治的80例SNIP患者作为研究对象,采用随机数表法将其分为两组,每组各40例。对照组予以低温等离子射频消融术治疗,观察组予以鼻内镜手术辅助低温等离子射频消融术治疗,比较两组患者鼻功能、复发率。结果:术后3个月,两组患者鼻腔最小横截面积(NMCA)、鼻腔容积(NCV)升高,鼻阻力降低,且观察组NMCA、NCV高于对照组,鼻阻力低于对照组,差异均有统计学意义(t=3.343、5.806、4.326,P<0.05);两组患者纤毛清除速度、黏液纤毛清除率升高,糖精清除时间减少,且观察组纤毛清除速度、黏液纤毛清除率高于对照组,糖精清除时间短于对照组,差异均有统计学意义(t=6.647、4.088、5.320,P<0.05);两组患者息肉、水肿、分泌物评分降低,且观察组息肉、水肿、分泌物评分低于对照组,差异均有统计学意义(t=4.420、8.854、5.935,P<0.05);两组患者生理问题、健康相关生活质量评分降低,且观察组生理问题、健康相关生活质量评分低于对照组,差异均有统计学意义(t=3.539、3.621,P<0.05)。随访1年发现,观察组术后复发率(2.50%)低于对照组(20.00%),差异有统计学意义(χ^(2)=4.507,P=0.034)。结论:鼻内镜手术辅助低温等离子射频消融术治疗对SNIP患者鼻功能改善效果更佳,且能降低复发率,减轻临床症状,提高生活质量。

Objective:To investigate the effect of nasal endoscopic radiofrequency plasma ablation on nasal function and recurrence rate in patients with nasal sinus inverted papilloma(SNIP).Methods:80 SNIP patients admitted to the hospital from March 2021 to March 2022 were selected according to the random number method.They were divided into 2 groups,with 40 patients each group.The control group was treated with cryogenic plasma radiofrequency ablation,and the observation group received nasal endoscopic surgery assisted with cryogenic plasma radiofrequency ablation.The nasal function and recurrence rate of the two groups were compared.Results:Min cross-section area(NMCA)and nasal volume(NCV),mucociliary clearance 3 months after surgery were higher in the observation group than those in the control group,and Nasal resistance and saccharin clearance time were decreased in all 2 groups,and those of the observation group is lower(t=3.343,5.806,4.326;P<0.05).Polyps,edema,and secretion scores were decreased(t=6.647、4.088、5.320,P<0.05).And those of the observation group was lower(t=4.420、8.854、5.935,P<0.05).The physiological problems and health-related quality of life scores were lower in the observation group than those in the control group,the difference was statistically significant(t=3.539,3.621;P<0.05).The postoperative recurrence rate of the observation group was lower than that of the control group,and the difference was statistically significant(χ^(2)=4.507,P=0.034).Conclusion:Nasal endoscopic surgery assisted by cryogenic plasma radiofrequency ablation can improve the nasal function of SNIP patients,reduce the recurrence rate and clinical symptoms,and improve the quality of life.
