本文使用SWOT分析法评估癌症末期病患在家中接受临终关怀时“家庭-社区-医疗机构”协同模式下的优点、缺点、机会与威胁。探讨肿瘤晚期患者“家庭安宁病床网格-社区医院安宁病床中心-医联体上级医院专科”的联动新模式(以下简称安宁疗护示范中心)。

This article utilizes the SWOT analysis method to evaluate the advantages,disadvantages,opportunities,and threats of"homecommunity-health institution"collaborative model for terminal cancer patients receiving hospice care at home.To explore a new linkage model of"home hospice bed grid,hospice bed center of community hospital and specialty of superior hospital of medical union"for advanced cancer patients(hereinafter referred to as the Hospice Centre of Excellence).
