门冬胰岛素联合盐酸二甲双胍治疗妊娠糖尿病的效果及对母婴结局的影响

Effect of Insulin Aspartate combined with Metformin Hydrochloride in the treatment of gestational diabetes mellitus and influence on maternal and infant outcomes

目的探讨门冬胰岛素联合盐酸二甲双胍治疗妊娠糖尿病的效果及对母婴结局的影响。方法选取聊城市第二人民医院2022年6月至2023年6月收治的86例妊娠糖尿病患者作为研究对象,以随机数字表法分为两组,每组43例。观察组予以门冬胰岛素联合盐酸二甲双胍治疗,对照组予以精蛋白生物合成人胰岛素治疗。比较两组血糖水平、不良反应发生情况、血糖达标时间、日胰岛素总量、分娩结局及妊娠不良结局、新生儿Apgar评分。结果治疗后,与对照组相比,观察组空腹血糖、餐后2小时血糖水平更低(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。观察组血糖达标时间短于对照组,日胰岛素总量低于对照组(P<0.05)。观察组剖宫产率及羊水过多、胎膜早破、早产发生率均低于对照组(P<0.05)。观察组新生儿Apgar评分高于对照组(P<0.05)。结论门冬胰岛素联合盐酸二甲双胍治疗妊娠糖尿病可降低血糖水平,改善妊娠结局,值得推广。

Objective To explore the effect of Insulin Aspartate combined with Metformin Hydrochloride in the treatment of gestational diabetes mellitus and influence on maternal and infant outcomes.Methods A total of 86 patients with gestational diabetes mellitus admitted to the Second People’s Hospital of Liaocheng from June 2022 to June 2023 were selected as the study objects.All the patients were divided into two groups by the random number table method,with 43 cases in each group.The observation group was treated with Insulin Aspartate combined with Metformin Hydrochloride,and the control group was treated with Isophane Protamine Biosynthetic Human Insulin.The blood glucose level,incidence of adverse reactions,time of reaching the standard of blood glucose,total daily insulin,delivery outcome,adverse pregnancy outcome and neonatal Apgar score were compared between the two groups.Results After treatment,compared with the control group,the observation group had lower levels of fasting plasma glucose and 2 hours postprandial blood glucose(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The time of reaching the standard of blood glucose in the observation group was shorter than that in the control group,and the total daily insulin was lower than that in the control group(P<0.05).The rates of cesarean section,polyhydramnios,premature rupture of membranes,and premature birth in the observation group were lower than those in the control group(P<0.05).The neonatal Apgar score in the observation group was higher than that in the control group(P<0.05).Conclusion The combination of Insulin Aspartate and Metformin Hydrochloride in the treatment of gestational diabetes mellitus can reduce the blood glucose level and improve pregnancy outcomes,which is worthy promotion.
