达格列净在老年2型糖尿病合并慢性心力衰竭患者中的应用效果

Efficacy of Dapagliflozin in Elderly Patients with Type 2 Diabetes Mellitus Combined with Chronic Heart Failure

目的:分析达格列净在老年2型糖尿病(T2DM)合并慢性心力衰竭(CHF)患者中的应用效果。方法:选取2021年1—7月天水市中西医结合医院收治的老年T2DM合并CHF患者124例为研究对象,随机分为参照组(常规治疗)和分析组(在参照组基础上给予达格列净治疗),各62例。比较两组生活质量评分、临床指标、再住院率和不良反应发生率。结果:治疗后,两组生活质量评分低于治疗前,分析组低于参照组(P<0.05);治疗后,两组糖化血红蛋白、空腹血糖、餐后2 h血糖、左心室舒张末期内径水平降低,左心室射血分数水平升高,分析组优于参照组(P<0.05);分析组再住院率低于参照组(P=0.028);分析组不良反应发生率低于参照组(P=0.015)。结论:达格列净在老年T2DM合并CHF患者中的应用效果确切,可改善患者心功能及血糖指标,降低再住院率和并发症发生率,提高生活质量。

Objective:To analyze the effect of dapagliflozin in elderly patients with type 2 diabetes mellitus(T2DM)combined with chronic heart failure(CHF).Methods:A total of one hundred and twenty-four elderly T2DM patients with CHF treated in Tianshui Hospital of Integrated Traditional Chinese and Western Medicine from January to July 2021 were selected as the study objects.They were randomly divided into the reference group(conventional treatment)and the analysis group(dapagliflozin treatment on the basis of the reference group),sixty-two cases each.Quality of life scores,clinical indicators,readmission rate and incidence of adverse reactions were compared between the two groups.Results:After treatment,the quality of life scores of the two groups were lower than before treatment,and the analysis group was lower than the reference group(P<0.05);after treatment,the levels of HBA1c,fasting blood glucose,2 h postprandial blood glucose and left ventricular end-diastolic diameter were decreased and left ventricular ejection fraction was increased in both groups,and the analysis group was better than the reference group(P<0.05);the readmission rate of analysis group was lower than that of reference group(P=0.028);the incidence of adverse reactions in analysis group was lower than that in reference group(P=0.015).Conclusion:The application of dapagliflozin in elderly T2DM patients with CHF has a definite effect,which can improve the cardiac function and blood glucose index of patients,reduce the rate of re-hospitalization and complications,and improve the quality of life.
