独活寄生汤辨证治疗膝关节骨性关节炎的效果观察

Effect of Duhuo Jisheng Decoction on Syndrome Differentiation of Knee Joint Osteoarthritis

目的:探讨独活寄生汤辨证治疗膝关节骨性关节炎(KOA)的效果。方法:选取2020年1月—2023年6月北京市延庆区延庆镇社区卫生服务中心收治的KOA患者100例作为研究对象,采用随机数字表法分为对照组和观察组,各50例。对照组采用常规治疗,观察组在对照组基础上采用独活寄生汤辨证治疗。比较两组治疗效果、关节腔液炎性因子水平、膝关节功能评分。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.021)。治疗前,两组白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05);治疗后,两组IL-1、IL-6、TNF-α水平均低于治疗前,观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组膝关节功能评分表(KSS)评分比较,差异无统计学意义(P>0.05);出院1个月后,两组KSS评分均高于治疗前,观察组高于对照组,差异有统计学意义(P<0.05)。结论:在常规治疗基础上联合独活寄生汤辨证治疗KOA的临床效果显著,有助于降低患者关节腔液炎性因子水平,改善患者膝关节功能。

Objective:To investigate the effect of Duhuo Jisheng Decoction on the treatment of knee joint osteoarthritis(KOA).Meth⁃ods:A total of one hundred KOA patients admitted to Beijing Yanqing District Yanqing Town Community Health Service Center from January 2020 to June 2023 were selected as the study objects,and were divided into control group and observation group by random number table method,with fifty cases in each group.The control group was treated with routine treatment,and the observation group was treated with Duhuo Jisheng Decoction based on the control group.The therapeutic effect,the level of inflammatory factors in joint fluid and the score of knee joint function were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.021).Before treatment,there was no significant difference in the levels of interleukin-1(IL-1),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05);after treatment,the levels of IL-1,IL-6 and TNF-αin 2 groups were lower than before treatment,and the obser⁃vation group was lower than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no sig⁃nificant difference in KSS scores between the two groups(P>0.05);one month after discharge,KSS scores in both groups were higher than before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:On the basis of conventional treatment,combined with Duhuo Jisheng Decoction has significant clinical effect in the treatment of KOA,which can help to reduce the level of inflammatory factors in joint fluid and improve the function of knee joint.
