全程化共享决策干预模式在环状混合痔患者术后的应用效果观察

Effect of Whole—Process Shared Decision Intervention Mode in Patients with Annular Mixed Hemorrhoids After Operation

目的:观察全程共享决策干预模式在环状混合痔患者术后的应用效果。方法:将我院2023年5~7月行环状混合痔手术38例患者随机分为对照组和观察组,每组19例。对照组患者实施常规护理干预,观察组患者实施全程化共享决策干预模式,比较2组患者术后康复情况、术后并发症发生率以及患者对医疗决策知情程度。结果:术后4h2组患者疼痛评分比较差异无统计学意义(P>0.05);术后48h2组患者疼痛评分均明显低于术后4h(P<0.05),且观察组低于对照组(P<0.05);创口愈合时间、便后出血时间及住院时间均短于对照组(P<0.05)。观察组患者术后并发症发生率明显低于对照组(P<0.05)。观察组患者医疗决策知情程度各项评分及总分均显著高于对照组(P<0.05)。结论:对环状混合痔手术患者采用全程化共享决策干预效果确切,可提高患者对医疗决策的知情度,有效减轻患者术后疼痛,缩短患者术后康复时间,同时降低术后并发症发生率。

Objective To observe the effect of whole-process shared decision intervention mode in pa-tients with annular mixed hemorrhoids after operation.Methods Total 38 patients who underwent an-nular mixed hemorrhoids operation in our hospital from May to July 2023 were randomly divided into con-trol group and observation group,with 19 cases in each group.Patients in the control group received rou-tine nursing intervention,and patients in the observation group received whole-process shared decision in-tervention mode.Postoperative rehabilitation,incidence of postoperative complications and patients'aware-ness of medical decision were compared between the two groups.Results There was no significant difference in pain score between the two groups at 4 h after operation(P>0.05).The pain score at 48 h after operation was significantly lower than that at 4 h after operation in both groups(P<0.05),and the pain score of the observation group was lower than that of the control group(P<0.05).Wound healing time,postoperative bleeding time and hospital stay of the observation group were shorter than those of the control group(P<0.05).The incidence of postoperative complications in the observation group was sig-nificantly lower than that in the control group(P<0.05).The scores and total scores of medical decision awareness in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The whole-process shared decision intervention for patients undergoing annular mixed hem-orrhoids operation is effective,which can improve patients'awareness of medical decision,effectively relieve patients'postoperative pain,shorten patients'postoperative rehabilitation time,and reduce the incidence of postoperative complications.
