结直肠癌组织中CHIP、FAT4表达及临床意义

摘要 目的探讨结直肠癌(CRC)组织中热休克蛋白70羧基末端相互作用蛋白(CHIP)、脂肪非典型钙黏蛋白4(FAT4)表达及临床意义。方法选取2018年5月至2020年5月在该院诊治的92例CRC患者作为研究对象。免疫组化检测CRC组织和癌旁组织中CHIP、FAT4表达。Spearman秩相关分析CRC组织中CHIP与FAT4表达的相关性。Kaplan-Meier曲线分析CHIP、FAT4表达与CRC患者生存预后的关系。Cox比例风险模型分析CRC患者预后影响因素。结果相比于癌旁组织,CRC组织中CHIP阳性率较高[65.22%(60/92)vs.10.87%(8/92)],FAT4阳性率较低[28.26%(26/92)vs.89.13%(82/92)],差异有统计学意义(χ^(2)=63.075、70.300,P<0.001)。CRC组织中CHIP与FAT4表达呈负相关(r=-0.781,P<0.001)。与TNM分期Ⅰ~Ⅱ期、高中分化、无淋巴结转移的CRC组织中CHIP、FAT4阳性率比较,TNM分期Ⅲ期、低分化程度及有淋巴结转移的CRC组织中CHIP阳性率较高,FAT4阳性率较低,差异有统计学意义(P<0.05)。CHIP阳性和CHIP阴性患者3年生存率分别为48.33%(29/60),78.13%(25/32),差异有统计学意义(Logrankχ^(2)=6.709,P=0.010)。FAT4阳性和FAT4阴性患者3年生存率分别为81.25%(13/16),53.95%(41/76),差异有统计学意义(Log-rankχ^(2)=5.124,P=0.032)。TNM分期Ⅲ期、低分化程度、有淋巴结转移、CHIP阳性是影响CRC患者预后的危险因素,FAT4阳性是预后的保护因素。结论CRC组织中CHIP表达升高,FAT4表达降低,两者表达与CRC不良临床病理特征有关,有助于评估CRC患者的生存预后。 Objective To investigate carboxyl terminus of Hsc70-interacting protein(CHIP)and FAT atypical cadherin 4(FAT4)expression in colorectal cancer(CRC)tissue and their clinical significance.Methods A total of 92 CRC patients treated in a hospital from May 2018 to May 2020 were selected as the study objects.The expressions of CHIP and FAT4 in CRC tissues and adjacent tissues were detected by immunohistochemistry.Spearman rank correlation analysis showed the correlation between CHIP and FAT4 expression in CRC tissues.Kaplan-Meier curve was used to analyze the relationship between CHIP and FAT4 expression and survival prognosis of CRC patients.Cox proportional risk model was used to analyze the prognostic factors of CRC patients.Results Compared with adjacent tissue,the positive rate of CHIP in CRC tissue was higher[65.22%(60/92)vs.10.87%(8/92)]and the positive rate of FAT4 was lower[28.26%(26/92)vs.89.13%(82/92)].The difference was statistically significant(χ^(2)=63.075,70.300,P<0.001).There was a negative correlation between CHIP and FAT4 expression in CRC(r=-0.781,P<0.001).The positive rates of CHIP and FAT4 were higher in CRC tissues with TNM stageⅠtoⅡ,high school differentiation and no lymph node metastasis,and the positive rates of CHIP and FAT4 were lower in CRC tissues with TNM stageⅢ,low differentiation and lymph node metastasis,and the difference was statistically significant(P<0.05).The 3-year survival rates of CHIP positive and CHIP negative patients were 48.33%(29/60)and 78.13%(25/32),respectively,and the difference was statistically significant(Log-rankχ^(2)=6.709,P=0.010).The 3-year survival rates of FAT4-positive and FAT4-negative patients were 81.25%(13/16)and 53.95%(41/76),respectively,with statistical significance(Log-rankχ^(2)=5.124,P=0.032).TNM stageⅢ,low differentiation,lymph node metastasis and positive CHIP were risk factors for the prognosis of CRC patients,while positive FAT4 was protective factor.Conclusion CHIP expression is increased and FAT4 expression is decreased in CRC tissues.Both expressions are associated with poor clinicopathological features of CRC,which is helpful to evaluate the survival prognosis of CRC patients.
