半夏泻心汤加减联合四联疗法治疗寒热错杂证慢性胃炎疗效观察及对胃蛋白酶的影响

Observation on Curative Effect of Modified Banxia Xiexin Decoction Combined with Quadruple Chemotherapy on Chronic Gastritis with Cold-Heat Complex Syndrome and Its Effecton Pepsin

目的:观察半夏泻心汤加减联合四联疗法治疗寒热错杂证慢性胃炎(CG)的临床效果及对胃蛋白酶的影响。方法:收集80例寒热错杂证CG患者临床资料,按随机数字表法分为对照组44例及观察组36例。对照组给予常规四联疗法治疗,观察组在对照组基础上给予半夏泻心汤治疗。治疗后,比较2组临床疗效、中医证候评分及胃蛋白酶指标[胃蛋白酶原Ⅰ(PGⅠ)及胃蛋白酶原Ⅱ(PGⅡ)]水平变化。结果:观察组总有效率为97.22%,对照组为81.82%,组间比较,差异有统计学意义(P<0.05)。治疗前,2组胃脘疼痛、胀满不适、嗳气不舒、口干口苦等中医证候评分比较,差异无统计学意义(P>0.05);治疗后,2组上述各项中医证候评分均较治疗前降低(P<0.05),且观察组各项评分均低于对照组(P<0.05)。治疗前,2组PGⅠ、PGⅡ水平比较,差异无统计学意义(P>0.05);治疗后,2组PGⅠ、PGⅡ水平均较治疗前降低(P<0.05),且观察组PGⅠ、PGⅡ水平均低于对照组(P<0.05)。治疗后,观察组幽门螺杆菌转阴率为88.89%,对照组为72.73%,组间比较,差异无统计学意义(P>0.05)。结论:半夏泻心汤加减联合四联疗法治疗寒热错杂证CG的疗效肯定,可有效改善患者中医证候,降低胃蛋白酶水平,促进胃黏膜修复。

Objective:To observe the clinical effect of modified Banxia Xiexin Decoction combined with quadruple chemotherapy on chronic gastritis(CG)with cold-heat complex syndrome and its effect on pepsin.Methods:The clinical data of 80 cases of patients with CG with cold-heat complex syndrome were collected and divided into the control group and the observation group according to the random number table method,with 44 and 36 cases in each group respectively.The control group was treated with routine quadruple chemotherapy,and the observation group was additionally treated with Banxia Xiexin Decoction based on the treatment of the control group.After treatment,the changes in clinical effects,traditional Chinese medicine(TCM)syndrome scores and levels of pepsin indexes[pepsinogenⅠ(PGⅠ)and pepsinogenⅡ(PGⅡ)]were compared between the two groups.Results:The total effective rate was 97.22%in the observation group and 81.82%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the scores of stomach duct pain,distension and discomfort,belching,dry mouth and bitter mouth between the two groups(P>0.05).After treatment,all the above TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and all the scores in the observation group were lower than those in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of the levels of PG-Ⅰand PG-Ⅱbetween the two groups(P>0.05).After treatment,the levels of PG-Ⅰand PG-Ⅱin the two groups were decreased when compared with those before treatment(P<0.05),and the levels of PG-Ⅰand PG-Ⅱin the observation group were lower than those in the control group(P<0.05).After treatment,the negative conversion rate of Helicobacter pylori in the observation group was 88.89%and 72.73%in the control group,the intergroup of comparison showed that there being no significance in the difference(P>0.05).Conclusion:Modified Banxia Xiexin Decoction combined with quadruple chemotherapy has a definite curative effect in the treatment of CG with cold-heat complex syndrome,which can effectively improve the TCM syndromes of patients,reduce the levels of pepsin,and promote the repair of gastric mucosa.
