经阴道实时三维子宫输卵管超声造影联合抗人绒毛膜促性腺激素抗体评估不孕症输卵管通畅程度的临床价值

摘要 目的探讨经阴道实时三维子宫输卵管超声造影(RT-3D-HyCoSy)联合抗人绒毛膜促性腺激素(HCG)抗体评估不孕症输卵管通畅程度的临床价值。方法选取2022年1—12月抚州市第一人民医院收治的120例不孕症患者作为研究对象,所有患者经腹腔镜通液术确诊,且均实施T-3D-HyCoSy检查,并检测患者血清抗HCG抗体水平。以腹腔镜通液术结果作为诊断金标准,比较RT-3D-HyCoSy、抗HCG抗体及RT-3D-HyCoSy联合抗HCG抗体检查对不孕症输卵管通畅程度的诊断效能。结果腹腔镜通液术诊断结果显示,120例(240条输卵管)不孕症患者中,输卵管通畅136条,占比56.67%;输卵管不通畅104条,占比43.33%。120例不孕症患者中,抗HCG抗体呈阳性者51例,阳性率为42.50%。RT-3D-HyCoSy联合抗HCG抗体诊断的灵敏度、特异度及准确度均高于RT-3D-HyCoSy、抗HCG抗体单一诊断,RT-3D-HyCoSy灵敏度、特异度及准确度均高于抗HCG抗体,差异有统计学意义(P<0.05)。结论临床上针对RT-3D-HyCoSy联合血清抗HCG抗体检查诊断不孕症输卵管通畅程度具有较高应用价值,可为临床医师诊疗提供更为有利且切实可靠的参考信息。 Objective Clinical value of real-time transvaginal three-dimensional hysterotubal contrast-enhanced ultrasound(RT-3D-HyCoSy)combined with anti-human chorionic gonadotropin(HCG)antibody in evaluating the patency of fallopian tubes in infertility.Methods 120 infertile patients in Fuzhou First People's Hospital from January to December 2022 were selected as the research subjects,al patients were diagnosed by lap-aroscopic hydrotubation,and T-3D-HyCoSy examination was performed,and the serum anti-HCG antibody level was detected.The results of laparo-scopic hydrotubation were used as the diagnostic gold standard to compare the diagnostic efficacy of RT-3D-HyCoSy,anti-HCG antibody and RT-3D-HyCoSy combined with anti-HCG antibody in the evaluating the patency of fallopian tubes in infertilit.Results The diagnostic results of lapa-roscopic hydrotubation showed that among 120 patients(240 fallopian tubes)with infertility,136 fallopian tubes were unobstructed,accounting for 56.67%;there were 104 obstructed fallopian tubes,accounting for 43.33%.Among the 120 infertility patients,51 were positive for anti-HCG anti-body,with a positive rate of 42.50%.The sensitivity,specificity and accuracy of RT-3D-HyCoSy combined with anti-HCG antibody diagnosis were higher than those of RT-3D-HyCoSy and anti-HCG antibody single diagnosis,the sensitivity,specificity and accuracy of RT-3D-HyCoSy were high-er than those of anti-HCG antibody,and the differences were statistically significant(P<0.05).Conclusion RT-3D-HyCoSy combined with anti-HCG antibody test is of high value in clinical diagnosis of patency of fallopian tubes in infertility,which can provide clinicians with more favorable and reliable data information.
