经结膜入路释放并保留眶隔脂肪眼袋整形术后的中面部年轻化效果评价

Evaluation of the effect of releasing and preserving orbital fat through conjunctival approach for rejuvenation of the mid face after blepharoplasty

目的评价经结膜入路释放并保留眶隔脂肪眼袋整形术后的中面部年轻化效果。方法回顾性分析焦作解放惠仁医院美容外科2022年3月至2023年3月收治的40例经结膜入路释放并保留眶隔脂肪眼袋整形术患者的临床资料。比较治疗前后患者的面部测量指标(瞳孔高度、容貌外眦高度、睑颊交界垂度、眉眼间距、眼裂高度)、中面部容积量表(middle facial volume scale,MMVS)、面部整体改善评价。结果40例患者中,治愈18例(45.00%),显效12例(30.00%),有效9例(22.50%),无效1例(2.50%),有效率97.50%。面部整体改善评价中,完全改善19例(47.50%),明显改善14例(35.00%),稍有改善7例(17.50%),无效0例。与术前相比,术后3个月的瞳孔高度显著增加,睑颊交界垂度显著降低,差异有统计学意义(P<0.05);术后3个月的容貌外眦高度、眉眼间距、眼裂高度与术前比较,差异无统计学意义(P>0.05)。术后3个月MMVS评分明显低于术前,差异有统计学意义(P<0.05)。本组40例患者发生肿胀1例(2.50%)。结论经结膜入路释放并保留眶隔脂肪眼袋整形术后的中面部年轻化效果显著,术中对眶隔脂肪合理在应用,更符合美学修复理念。

Objective To evaluate the effect of mid face rejuvenation after conjunctival release and preservation of orbital fat eye bags for plastic surgery.Methods A retrospective analysis was conducted on the clinical data of 40 patients who underwent conjunctival approach to release and preserve orbital fat eye bags in the Department of Cosmetic Surgery at Jiaozuo Jiefang Huiren Hospital from March 2022 to March 2023.Compare the facial measurement indicators of patients before and after treatment,including pupil height,outer canthus height,droop of eyelid cheek junction,distance between eyebrows and eyes,and eye fifissure height,as well as the middle facial volume scale(MMVS)and Global Aesthetic Improvement Scale.Results Among the 40 patients,18 were cured(45.00%),12 showed significant improvement(30.00%),9 were effective(22.50%),and 1 was ineffective(2.50%),with an effective rate of 97.50%.In the evaluation of overall facial improvement,19 cases(47.50%)showed complete improvement,14 cases(35.00%)showed signifificant improvement,7 cases(17.50%)showed slight improvement,and 0 cases were ineffective.Compared with before surgery,the pupil height signifificantly increased and the droop of the eyelid cheek junction signifificantly decreased at 3 months after surgery,with statistical significance(P<0.05).However,there was no statistically significant difference in the height of the outer canthus,distance between eyebrows and eyes,and height of the eye fifissure at 3 months after surgery compared with before surgery(t-values were,,,P values were,,,respectively).Three months after surgery,the MMVS score was signifificantly lower than before surgery,and the difference was statistically signifificant(P<0.05).One of the 40 patients in this group(2.50%)experienced swelling.Conclusions The release and preservation of orbital fat through the conjunctival approach after blepharoplasty has a signifificant effect on rejuvenation of the mid face.The reasonable application of orbital fat during surgery is more in line with aesthetic restoration concepts.
