基于适时模式的出院准备服务在老年慢性阻塞性肺疾病患者中的应用效果

Application effects of discharge preparation service based on timely mode in elderly patients with chronic obstructive pulmonary disease

目的:观察基于适时模式的出院准备服务在老年慢性阻塞性肺疾病(COPD)患者中的应用效果。方法:选取2020年7月至2022年8月该院收治的80例老年COPD患者进行前瞻性研究,按照随机数字表法将其分为观察组和对照组各40例。对照组实施常规护理,观察组在对照组基础上实施基于适时模式的出院准备服务。比较两组出院准备度[出院准备度量表(RHDS)]评分,护理前后照顾者照护能力[家庭照顾者照顾能力量表(FCTI)]评分、生命质量[生活质量综合评定问卷-74(GQOLI-74)]评分,以及护理满意度。结果:观察组自身状况、院外应对能力、疾病知识、预期社会支持等RHDS各维度评分均高于对照组,差异有统计学意义(P<0.05);护理后,观察组调整个人及照顾需求、适应照顾角色、应变及提供协助、评估家人及社区资源、处理个人情绪需要等FCTI各维度评分均低于对照组,差异有统计学意义(P<0.05);护理后,观察组心理功能、躯体功能、社会功能、物质生活状态等GQOLI-74各维度评分均高于对照组,差异有统计学意义(P<0.05);观察组护理满意度为95.00%(38/40),高于对照组的80.00%(32/40),差异有统计学意义(P<0.05)。结论:基于适时模式的出院准备服务应用于老年COPD患者可提高出院准备度、生命质量评分和护理满意度,降低照顾者照护能力评分,效果优于常规护理。

Objective:To observe application effects of discharge preparation service based on timely mode in elderly patients with chronic obstructive pulmonary disease(COPD).Methods:A prospective study was conducted on 80 elderly patients with COPD admitted to the hospital from July 2020 to August 2022.According to the random number table method,they were divided into observation group and control group,40 cases in each group.The control group received routine nursing,while the observation group received the discharge preparation service based on timely mode on the basis of that of the control group.The discharge readiness[readiness for hospital discharge scale(RHDS)]score,the caregiver care ability[family caregiver task inventory(FCTI)]score,the quality of life[generic quality of life inventory-74(GQOLI-74)]score before and after the nursing,and the nursing satisfaction were compared between the two groups.Results:The scores of RHDS dimensions such as self-status,out-of-hospital coping ability,disease knowledge and expected social support in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the nursing,the scores of FCTI dimensions such as adjusting personal and care needs,adapting to care roles,responding and providing assistance,assessing family and community resources,and dealing with personal emotional needs in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the nursing,the scores of GQOLI-74 dimensions such as psychological function,physical function,social function and material life status in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the nursing satisfaction of the observation group was 95.00%(38/40),which was higher than 80.00%(32/40)of the control group,and the difference was statistically significant(P<0.05).Conclusions:The discharge preparation service based on timely mode in the elderly patients with COPD can improve the discharge preparation scores,the quality of life scores and the nursing satisfaction,and reduce the caregiver care ability scores.Moreover,it is superior to the routine nursing.
