多发性骨髓瘤染色体不稳定性的研究现状

摘要 多发性骨髓瘤(MM)是一种以染色体不稳定性(CIN)为特征的浆细胞恶性肿瘤。CIN是指细胞在连续分裂过程中,染色体错误分离的几率增加,进而引发MM细胞遗传学异常与克隆演化,最终影响患者预后结局。目前越来越多的临床与基础研究关注MM相关遗传学变异,并尝试解释MM患者的CIN分子机制及其与耐药和临床转归的相关性。笔者拟就MM患者的CIN发生情况及其分子基础,以及CIN与MM患者治疗相关性的研究现状进行阐述,旨在加深临床对MM患者CIN的理解,为开发更有效的治疗策略、克服耐药提供新的思路。 Multiple myeloma(MM)is a plasma cell malignancy characterized by chromosomal instability(CIN).CIN refers to increased probability of chromosomal missegregation during successive cell divisions,which in turn triggers cytogenetic abnormality with clonal evolution and ultimately affects prognostic outcome of patients.Currently,more and more clinical and basic studies are focusing on MM-associated genomic variants and trying to explain the molecular mechanisms of CIN,treatment resistances and clinical regressions in MM patients.This article intends to elaborate on the research status of occurrence and molecular basis of CIN in MM patients,as well as correlation between CIN and treatment of MM patients,aiming to deepen the understanding of CIN in MM and provide new ideas for developing more effective therapeutic strategies and overcoming drug resistance.
