血清和胸腔积液TSGF、OSM水平对恶性胸腔积液的诊断价值

摘要 目的:探讨肿瘤特异性生长因子(TSGF)、抑瘤素M(OSM)水平对恶性胸腔积液(MPE)的诊断价值。方法:选取2021年2月至2023年2月在本院进行治疗的172例渗出性胸腔积液患者,根据检查结果将患者分为MPE组和良性胸腔积液(BPE)组。收集患者一般资料并采血检测相关生化指标;采用酶联免疫法检测血清及胸腔积液(PE)中的TSGF、OSM水平,多因素Logistic分析MPE发生的影响因素,受试者工作特征(ROC)曲线分析TSGF、OSM水平对MPE的诊断价值。结果:两组乳酸脱氢酶(LDH)、胸腔积液腺苷脱氨酶(PEADA)水平比较,差异均有统计学意义(P<0.05);与BPE组相比,MPE组患者血清及PE中的TSGF、OSM水平均显著升高(P<0.05);多因素Logistic分析结果显示LDH、PEADA、血清及PE中的TSGF、OSM水平均是影响MPE发生的因素(P<0.05);血清TSGF、OSM及两者联合诊断MPE的曲线下面积(AUC)分别为0.798、0.855、0.925,两者联合诊断的灵敏度和特异度分别为88.30%和84.62%,两者联合诊断价值高于单独诊断(Z_(两者联合-血清TSGF)=4.275,Z_(两者联合-血清OSM)=3.297,均P<0.05);PETSGF、PEOSM及两者联合对MPE诊断的AUC分别为0.886、0.765、0.921,两者联合诊断的灵敏度和特异度分别为90.42%和79.48%,两者联合诊断价值高于单独诊断(Z_(两者联合-PETSGF)=4.275,Z_(两者联合-PEOSM)=3.297,均P<0.05)。结论:MPE患者血清及PE中的TSGF、OSM水平升高,两个指标联合诊断MPE具有较高的临床价值。 Objective:Exploring the diagnostic value of tumor specific growth factor(TSGF)and oncostatin M(OSM)levels in malignant pleural effusion(MPE).Methods:One hundred and seventy-two patients with exudative pleural effusion(PE)who underwent treatment in our hospital from February 2021 to February 2023 were selected as the study subjects,and the patients were divided into the MPE group and the benign pleural effusion(BPE)group according to the results of the examination after admission.General data of patients were collected,and blood was collected to test relevant biochemical indexes;TSGF and OSM levels in serum and PE were detected by enzyme immunoassay;multifactorial Logistic analysis was performed to analyze the influencing factors of the occurrence of MPE;and ROC curves were performed to analyze the diagnostic value of TSGF and OSM levels on MPE.Results:Lactate dehydrogenase(LDH)and pleural effusion adenosine deaminase(PEADA)levels in the MPE group compared with those in the BPE group showed statistically significant differences(P<0.05);TSGF and OSM levels in serum and PE were significantly higher in patients in the MPE group compared with those in the BPE group(P<0.05);and the results of the multifactorial Logistic analysis showed that LDH,PEADA,TSGF and OSM levels in serum and PE were all factors influencing the occurrence of MPE(P<0.05).The area under the curve(AUC)of serum TSGF,OSM,and their combined diagnosis for MPE was 0.798,0.855,and 0.925,respectively.The sensitivity and specificity of their combined diagnosis were 88.30%and 84.62%,respectively.The combined diagnostic value of the two was higher than that of their individual diagnosis(Z_(combination-serum TSGF)=4.275,Z_(combination-serum OSM)=3.297,all P<0.05).The AUC of PETSGF,PEOSM,and their combination for the diagnosis of MPE was 0.886,0.765,and 0.921,respectively.The sensitivity and specificity of their combined diagnosis were 90.42%and 79.48%,respectively.The combined diagnostic value of PETSGF,PEOSM,and their combination was higher than their individual diagnosis(Z_(combination-PETSGF)=4.275,Z_(combination-PEESM)=3.297,all P<0.05).Conclusion:The levels of TSGF and OSM in the serum and PE of MPE patients are elevated,and the combination of these two indicators has high clinical value in diagnosing MPE.
