【目的】了解2017—2020年上海市道路交通伤害住院费用及影响因素,为今后开展道路交通伤害预防提供依据。【方法】采集2017—2020年上海市22家试点开展伤害住院登记报告工作的医院中因道路交通伤害住院病例信息。采集的信息内容主要包括基本人口学信息、伤害住院总费用、交通方式、伤害原因、伤害性质、伤害部位、医院等级,单因素方差分析不同年份间平均住院费用和平均住院天数差异,Kruskal-Wallis检验分析不同年份间住院费用、住院天数、不同人群特征住院费用的差异。采用全子集回归模型探讨住院费用的主要影响因素。【结果】2017—2020年因道路交通伤害入院治疗共4655人,其中男性2675人(57.47%),女性1980人(42.53%)。年龄最小4岁,最大97岁,平均(53.84±15.37)岁。非机动车祸1907人(40.97%),机动车祸2748人(59.03%)。平均住院费用为23245.71元,中位数为9793.94元;平均住院天数为13.61 d,中位数为10.00 d。住院费用影响因素主要包括年份、伤害性质、伤害部位、住院天数和医院级别。【结论】道路交通伤害严重影响中青年人群健康,尤其对行人和骑车者等弱势群体造成严重的后果,导致直接经济负担逐年增高,应积极开展道路交通伤害预防以减少伤害的发生。

[Objective]To assess the direct economic burden and influencing factors of road traffic injuries in Shanghai,providing a basis for future prevention of road traffic injuries.[Methods]Information of inpatient cases caused by road traffic injuries was collected from 22 pilot hospitals in Shanghai that participated in the registration and reporting system of injury hospitalizations from 2017 to 2020.The main content of the collected information included basic demographic information,total hospitalization costs for injuries,mode of transportation,cause of injury,nature of injury,location of injury,and hospital level.Univariate analysis was used to analyze the average hospitalization costs and length of hospital stay between different years.Kruskal-Wallis test was used to analyze the median hospitalization costs and the median length of hospital stay between different years,and the median hospitalization costs of different groups.A full subset regression model in multivariate regression was used to explore the main influencing factors of hospitalization costs.[Results]A total of 4655 patients were hospitalized with road traffic injuries,including 2675 males(57.47%)and 1980 females(42.53%).The average age was(53.84±15.37)years old,with a minimum age of 4 years old and a maximum age of 97 years old.1907 patients(40.97%)had non-motor vehicle accidents,and 2748 patients(59.03%)had motor vehicle accidents.The average hospitalization cost is 23245.71 yuan,with a median of 9793.94 yuan.The average length of stay was 13.61 days,with a median stay of 10.00 days.The main factors affecting hospitalization costs included year,nature of injury,location of injury,length of hospital stay,and hospital level.[Conclusion]Road traffic injuries seriously affect the health of middleaged and young people,especially vulnerable road user such as pedestrians and cyclists,causing serious injury and an increase of direct economic burden year by year.Therefore,road traffic injury prevention projects should be actively carried out to reduce the occurrence of injuries.
