帕金森病患者血压变化与认知功能的研究进展

Research progress on blood pressure changes and cognitive function in Parkinsons patients

帕金森病(PD)是临床常见的神经系统退行性改变疾病,涉及多个神经区域,典型运动障碍是患者的主要临床症状,同时还伴随一些非运动症状,包括血压异常、认知功能障碍等。血压异常变化包括直立性低血压、仰卧位高血压、血压节律异常等,往往导致患者突然晕厥,增加跌伤风险。认知功能障碍可发生在PD任一阶段,并呈进行性加重,患者以记忆、语言、视空间功能障碍为主要表现,影响患者日常生活与工作。随着病情进展,这些非运动症状可贯穿疾病全程,不仅极大影响患者生活质量,也加大了陪护人员的负担,是当前临床研究关注的重点。本文就PD患者血压变化与认知功能障碍的发病机制、影响因素、管理与治疗方面的进展作一综述。

Parkinsons disease(PD)is a common clinical degenerative disease of the nervous system,involving multiple nerve regions,typical movement disorders are the main clinical symptoms of patients,but also accompanied by some non-motor symptoms,including abnormal blood pressure,cognitive dysfunction,etc.Abnormal changes in blood pressure include orthostatic hypotension,supine hypertension,abnormal blood pressure rhythm,etc.,which often lead to sudden syncope and increase the risk of falls.Cognitive dysfunction can occur at any stage of PD and is progressively aggravated,with memory,language,and visuospatial dysfunction as the main manifestations,affecting patientsdaily life and work.With the progression of the disease,these non-motor symptoms can run through the whole course of the disease,which not only greatly affects the quality of life of patients,but also increases the burden of accompanying personnel,which is the focus of current clinical research.This article reviews the pathogenesis,influencing factors,management and treatment of blood pressure changes and cognitive dysfunction in PD patients。
