个性数字化辅助椎体成形术单侧穿刺治疗胸腰椎骨质疏松性骨折的疗效分析

Analysis of the efficacy of personalized digital-assisted percutaneous vertebroplasty with unilateral puncture in the treatment of thoracolumbar osteoporotic fractures

摘要 目的探讨术前CT个性数字化辅助椎体成形术单侧穿刺治疗胸腰椎骨质疏松性椎体压缩骨折的临床疗效。方法回顾性分析自2020年1月至2023年1月诊治的53例胸腰椎骨质疏松性椎体压缩骨折,28例采用个性数字化设计辅助单侧穿刺PVP治疗(单侧组),25例采用常规双侧PVP治疗(对照组)。比较2组患者手术时间、术中透视次数、骨水泥注入量、骨水泥渗漏率,比较2组术前、术后1 d、术后3个月、末次随访时的疼痛VAS评分、ODI评分以及伤椎Cobb角。结果各组均顺利完成手术。单侧组手术时间、术中透视次数、骨水泥注入量、骨水泥渗漏率较对照组少,差异差异有统计学意义(P<0.05)。2组术前、术后1 d、术后3个月、末次随访时的疼痛VAS评分、ODI评分以及伤椎Cobb角比较,差异无统计学意义(P>0.05)。结论个性数字化辅助单侧穿刺PVP可减少术中透视次数和手术时间,提高穿刺准确性和减少相关并发症,临床疗效满意。 Objective To explore the clinical efficacy of preoperative CT-based personalized digital-assisted percutaneous vertebroplasty with unilateral puncture in the treatment of thoracolumbar osteoporotic vertebral compression fractures.Methods A retrospective analysis was performed on 53 cases of thoracolumbar osteo-porotic vertebral compression fractures treated from January 2020 to January 2023.28 patients received personalized digital design-assist-ed unilateral puncture PVP(the observation group),while the other 25 patients received conventional bilateral puncture PVP(the control group).The operation duration,the number of intraoperative fluoroscopy,the amount of bone cement injection and the bone cement leak-age rate were compared between the two groups.The pain VAS scores,ODI scores and Cobb Angle of the injured vertebra were com-pared between the two groups before surgery,1 day after surgery,3 months after surgery,and at the last follow-up.Results The operation was successfully completed in both groups.The operation duration,the number of intraoperative fluoroscopy,the amount of bone cement injection and the bone cement leakage rate in the observation group were significantly lower than those in the control group(P<0.05).There were no significant differences in VAS score,ODI scores and Cobb Angle of injured vertebra between the two groups before surgery,1 day after surgery,3months after surgery,and at the last follow-up(P>0.05).Conclusion Personalized digital-assisted unilateral puncture PVP can reduce the number of intraoperative fluoroscopy and the operation duration,improve puncture accuracy and reduce related complications,with satisfactory clinical efficacy.
