“推肩俯卧撑”配合运动疗法在肩峰撞击综合征康复中的疗效观察

Observation on the Curative Effect of"Shoulder Push-ups"Combined with Kinesitherapy in the Rehabilitation of Subacromial Impingement Syndrome

摘要 目的观察采用“推肩俯卧撑”配合运动疗法在肩峰撞击综合征康复中的疗效,为优化肩峰撞击综合征康复治疗方案提供参考。方法选取2021年10月至2023年9月在成都市第一骨科医院、成都市第二人民医院康复医学科诊断为肩峰撞击综合征患者共60例,但在实际试验中排除患者不能坚持治疗、治疗结束时未及时测试等情况,最终实际纳入患者共53例,将患者随机分为试验组(n=26)和对照组(n=27),两组均给予物理因子、手法治疗等常规康复治疗,对照组在此基础上进行运动疗法,试验组则采用“推肩俯卧撑”配合运动疗法。治疗前后均使用视觉模拟评分(VAS)对肩关节的疼痛程度进行评分,采用肩关节的主动活动测量(AROM)对肩关节屈曲和外展活动度进行评估;此外,还引入肩关节疼痛及功能障碍指数(SPADI)对肩关节的疼痛和功能障碍进行评分。结果治疗6周后,两组患者VAS评分、肩关节主动屈曲和外展活动度以及SPADI评分均得到了改善(P<0.05)。试验组VAS评分、AROM(外展)角度、SPADI评分及AROM(屈曲)角度的结果优于对照组(P<0.05)。结论临床在肩峰撞击综合征患者治疗中,采用“推肩俯卧撑”配合运动疗法能更有效地缓解患者肩痛、改善肩关节屈曲和外展主动活动度,进而促进肩关节功能恢复。 Objective To explore the effect of"shoulder push-ups"combined with kinesitherapy in the rehabilitation of subacromial impingement syndrome(SIS),so as to provide reference for optimizing the rehabilitation treatment of SIS.Merhods A total of 60 patients diagnosed with SIS in NO.1 Orthopedics Hospital of Chengdu and the Rehabilitation Medicine Department of Chengdu Second People's Hospital from October 2021 to September 2023 were selected.However,only 53 patients were included,since patients who did not finish the treatment or take the test in time after treatment were excluded.They were randomly divided into an experimental group(n=26)and a control group(n=27).Both groups were given routine rehabilitation therapy such as physical therapy and manual therapy.On this basis,the control group was given kinesitherapy while the experimental group was given kinesitherapy combined with"shoulder push-ups".Visual Analogue Scale(VAS)was used to evaluate the degree of shoulder pain before and after treatment.Active Range of Motion(AROM)was used to evaluate shoulder joint flexion and abductive mobility.And Shoulder Pain and Disability Index(SPADI)was used to evaluate shoulder pain and dysfunction.Results After 6 weeks of treatment,the VAS score,active shoulder flexion and abductive mobility,and SPADI score in both groups were significantly improved(P<0.05).Compared with the control group,the experimental group had increased VAS score,AROM(abduction)angle and SPADI score(P<0.05),and elevated AROM(flexion)angle(P<0.05).Conclusion"Shoulder push-ups"combined with kinesitherapy can effectively relieve shoulder pain,improve shoulder joint flexion and abductive mobility,and thus promote the recovery of shoulder joint function in the clinical treatment of patients with SIS.
