摘要
目的应用体素内不相干运动扩散加权成像研究正常胰腺及急性胰腺炎的不同,探讨正常胰腺及急性胰腺炎的胰腺不同解剖部位参数差异及意义。方法29名健康志愿者及33例AP患者行上腹部MRI及IVIM-DWI扫描,测量胰腺头部、颈体部、尾部ADC、f、D、D*值。采用单因素方差、LSD检验和独立样本t检验对比分析正常胰腺及AP胰腺参数差异性。结果正常胰腺不同解剖部位ADC值及f值差异显著,病变为弥漫AP者不同解剖部位各参数差异无统计学意义。AP患者ADC、f、D值均低于正常胰腺,D值高于正常胰腺,差异均有统计学意义(P<0.05)。病变为局限AP患者ADC及f值由高到低排序为NP组>非病变区>病变区,差异有统计学意义(P<0.05)。结论胰腺不同解剖部位ADC、f、D、D*值有作为甄别急性胰腺炎指标的可能,其中f值是间接评估胰腺不同解剖部位微循环灌注情况的主要参数。
Objective Study the difference by the application of voxel incoherent motion diffusion-weighted imaging for normal pancreas and acute pancreatitis,and explore the difference and significance of parameters in different anatomical parts of normal pancreas and acute pancreatitis.Methods 29 healthy volunteers and 33 acute pancreatitis patients underwent upper abdominal MRI and IVIM-DWI scanning to measure ADC,f,D and D values of the head,neck and tail of the pancreas.Single factor variance,LSD test and independent sample t-test were used to compare and analyze the difference between normal pancreas and AP pancreas parameters.Results There were significant differences in ADC and f-values among different anatomical parts of normal pancreas,but no statistically significant difference in the parameters of different anatomical parts of diffuse AP patients.The ADC,f and D values in AP patients were lower than those in normal pancreas,and D*values were higher than those in normal pancreas,and the differences were statistically significant(P<0.05).The ADC and f-values of patients with limited AP were ranked from high to low as follow:NP group>non-lesion part>lesion part,with statistically significant differences.Conclusions The ADC,f,D and D values in different anatomical parts of pancreas may be used as indicators for identification of acute pancreatitis,in which f value is the main parameter for indirect evaluation of microcirculation perfusion in different anatomical parts of pancreas.
