广州地区女性人乳头瘤病毒感染及宫颈细胞学检测结果分析

摘要 目的对门诊女性人乳头瘤病毒(human papillomavirus,HPV)感染及宫颈细胞学检测结果进行分析,为宫颈癌的预防提供科学依据。方法回顾性分析2022年7-12月在广州市某三甲医院妇科门诊就诊女性的HPV、HPV亚型及宫颈细胞学检测结果。结果研究共纳入3573名就诊女性,HPV总感染率为21.66%(774/3573),以高危基因型(15种HPV)为主,感染率为18.58%(664/3573),低危基因型(6种HPV)感染率为5.46%(195/3573)。HPV感染类型以单一感染为主,感染率最高的5种高危基因型分别是HPV52、16、39、58、51,最高的低危基因型为HPV81。不同年龄组HPV总感染率、高危基因型感染率、低危基因型感染率均呈U型分布,差异有统计学意义(均P<0.001)。宫颈细胞学异常总检出率为14.05%,其中不典型鳞状上皮细胞检出率为10.13%、低度鳞状上皮内病变检出率为3.44%、高度鳞状上皮内病变检出率为0.48%。细胞学异常女性的HPV总感染率、高危基因型感染率、低危基因型感染率均高于细胞学正常女性,差异有统计学意义(均P<0.001)。结论广州地区门诊女性HPV感染以单一感染为主,最常见的感染亚型是HPV52、16、39、58、51,宫颈细胞学病变与HPV感染有关,应根据女性不同年龄HPV感染率和感染型别制定适宜的宫颈癌预防策略。 Objective To analyze the results of human papillomavirus(HPV)infection and cervical cytology test(CCT)in outpatients,and to provide scientific basis for the prevention of cervical cancer.Methods We retrospectively analyzed the HPV infection rate and CCT outcomes in gynecological outpatients of a tertiary hospital in Guangzhou from July to December 2022.Results A total of 3573 women outpatients were included in the analysis.The total infection rate of HPV was 21.66%(774/3573),the infection rate of high-risk genotype(16 types of HPV)was 18.58%(664/3573),and the infection rate of low-risk HPV(6 types of HPV)was 5.46%(195/3573).Most patients infected by one subtype of HPV,and the five high-risk genotypes with the highest infection rate were HPV52,16,39,58,51,and the highest low-risk genotype was HPV81.The total infection rate of HPV,high-risk gene infection rate and low-risk gene infection rate in different age groups showed U-shaped distribution,and the differences were statistically significant(P<0.001).14.05%woman were CCT positive,of whom 10.13%,3.44%,and 0.48%had atypical squamous cells,low-grade squamous intraepithelial lesions and high-grade squamous intraepithelial lesions,respectively.The HPV infection rate,high-risk gene infection rate and low-risk gene infection rate in women with abnormal cytology were higher than those in women with normal cytology,and the differences were statistically significant(P<0.001).Conclusion The most common subtypes of HPV infection are HPV52,16,39,58,51.Cervical cytological lesions are related to HPV infection.Appropriate cervical cancer prevention strategies should be developed according to the HPV infection rate and infection type of women at different ages.
