总结分析医院-社康-社区-居民四方联动模式下1例慢性阻塞性肺疾病终末期患者居家安宁疗护的实践经验。基于四方联动健康新模式及互联网+居家照护平台的建立,通过氧疗护理、休息及活动指导、饮食指导、用药指导协同促进患者躯体舒适;在多学科合作下运用中医特色音乐疗法、支持性心理护理及共情护理、陪伴式人生回顾、签署生前预嘱、哀伤辅导,给予患者及家属灵性照护。经为期6个多月的居家照护,患者在其家属的陪伴下平静离世,患者的家属坦然渡过悲伤期,实现生死两相安,取得较为满意的护理效果。

To summarize and analyze the practice experience of home hospice care for a patient with end-stage chronic obstructive pulmonary disease under the four-way linkage model involving hospital-community health services-community-residents.Based on the new model of the four-way linkage model in health care and the establishment of an Internet-based home care platform,the practice promotes physical comfort for the patient through oxygen therapy,rest and activity guidance,dietary advice,and medication guidance.Utilizing multidisciplinary cooperation,the approach incorporates traditional Chinese medicine music therapy,supportive psychological care with empathy,life review with companionship,advance directives,and grief counseling to provide spiritual care for both the patient and their family members.After over six months of home care,the patient passed away 7peacefully with their family by their side,and the family members managed to move through the grieving process calmly,achieving peace with life and death,resulting in a satisfactory nursing outcome.
