行为激活干预对中青年脑卒中家庭照顾者病耻感的影响

Effect of behavioral activation intervention on stigma of young and middle-aged stroke family caregivers

目的探讨行为激活干预在中青年脑卒中家庭照顾者中的应用效果。方法选取2022年7月至2023年7月广东省工伤康复医院进行康复治疗的60例中青年脑卒中患者及其家庭照顾者作为研究对象,按照随机数字表法分为对照组(30例)和试验组(30例)。对照组对患者采取常规干预,试验组在常规干预基础上实施行为激活疗法干预,两组均持续干预4周。比较两组家庭照顾者病耻感、负性情绪、患者日常生活质量以及家庭照顾者满意度。结果试验组干预后家庭照顾者慢性病病耻感量表(SSCI)各项评分、抑郁自评量表(PHQ-9)评分均低于对照组,差异有统计学意义(P<0.05);试验组干预后患者脑卒中专用生活质量量表(SS-QOL)评分高于对照组,差异有统计学意义(P<0.05);试验组家庭照顾者满意度各项评分均高于对照组,差异有统计学意义(P<0.05)。结论行为激活干预可减轻中青年脑卒中家庭照顾者病耻感,有效调节其不良情绪,促使其以更加积极的心态照顾患者,促进患者生活质量提升,从而获得更高的家庭照顾者满意度。

Objective To explore the application effect of behavioral activation intervention in young and middle-aged stroke family caregivers.Methods A total of 60 young and middle-aged stroke patients and their family caregivers who received rehabilitation treatment in Guangdong Work Injury Rehabilitation Hospital from July 2022 to July 2023 were selected as the research objects.According to the random number table method,they were divided into the control group(30 cases)and the experimental group(30 cases).The control group received routine intervention,and the experimental group received behavioral activation therapy intervention on the basis of routine intervention.Both groups continued to intervene for 4 weeks.The stigma,negative emotions,quality of daily life and satisfaction of family caregivers were compared between the two groups.Results The scores of stigma scale for chronic illness(SSCI)and patient health questionnaire-9(PHQ-9)of family caregivers in test groups were lower than those in control group after intervention,and the differences were statistically significant(P<0.05).The score of stroke specific quality of life scale(SS-QOL)in experimental group was higher than that in control group after intervention,and the difference was statistically significant(P<0.05).The score of family caregiver satisfaction in experimental group was higher than that in control group,and the difference was statistically significant(P<0.05).Conclusion Behavioral activation intervention can reduce the stigma of middle-aged and young stroke family caregivers,effectively regulate their negative emotions,and encourage them to take care of patients with a more positive attitude,improve the quality of life of patients,and thus obtain higher satisfaction of family caregivers.
