超声骨刀联合骨皮质切开术牵引上颌埋伏尖牙的临床研究

Clinical Study of Ultrasonic Osteotome Combined With Corticotomy in Traction of Maxillary Impacted Canines

目的探究超声骨刀联合骨皮质切开术在上颌埋伏尖牙正畸牵引治疗中的临床应用效果。方法选取2020年7月—2023年9月于厦门医学院附属口腔医院需行正畸牵引治疗的上颌埋伏尖牙64例患者作为研究对象,采用随机数表法将患者分为对照组和试验组,各32例。对照组采用45°仰角涡轮联合骨皮质切开术,试验组采用超声骨刀联合骨皮质切开术。比较2组治疗总有效率、术中心理状态、术后疼痛、脸部肿胀程度。结果试验组的治疗总有效率为100%,高于对照组的96.88%,差异无统计学意义(P=1.000)。试验组术中心理状态优于对照组,差异有统计学意义(P=0.009)。试验组患者术后24 h疼痛程度轻于对照组,术后48 h脸部肿胀程度轻于对照组,差异有统计学意义(P<0.05)。结论采用超声骨刀联合骨皮质切开术对上颌埋伏尖牙进行正畸牵引治疗虽未能显著提高治疗总有效率,但能有效缓解患者的心理压力,减轻术后疼痛及脸部肿胀症状,具有进一步研究的价值。

Objective To explore the clinical effect of ultrasonic osteotome combined with corticotomy in orthodontic traction treatment of maxillary impacted canines.Methods A total of 64 patients diagnosed with maxillary impacted canines requiring orthodontic traction treatment from July 2020 to September 2023 in Stomatological Hospital of Xiamen Medical College were selected as the research subjects.The patients were randomly divided into control group and testing group,with 32 cases in each group.The control group underwent bone corticotomy using a 45°angulated turbine handpiece,while the testing group underwent bone corticotomy using ultrasonic osteotome.The results of the total effective rate,psychological state,postoperative pain,swelling degree of face were compared between the two groups.Results The total effective rate of treatment in the experimental group was 100%,higher than that in the control group,which was 96.88%,with no statistically significant difference(P=1.000).The psychological state during surgery in the experimental group was better than that in the control group,with a statistically significant difference(P=0.009).The pain level in the experimental group was lower than that in the control group 24 hours after surgery,and the facial swelling level was less server than that in the control group 48 hours after surgery,with a statistically significant difference(P<0.05).Conclusion Although the orthodontic traction treatment of maxillary ambushing fangs by Ultrasound osteoto me combined with corticotomy could not significantly improve significantly effect,it could not improve significantly the total effective rate of treatment,but can effectively relieve the psychological pressure on patients,and relieve postoperative pain and facial swelling symptoms,which is worthy of popularization and application.
