益肾化瘀降浊方联合腹膜透析治疗对慢性肾功能衰竭患者残余肾功能影响

Influence of Yishen Huayu Jiangzhuo decoction combined with peritoneal dialysis on residual renal function in patients with chronic renal failure

目的:探究益肾化瘀降浊方联合腹膜透析(PD)治疗血瘀湿浊证慢性肾功能衰竭(CRF)患者残余肾功能(RRF)的影响。方法:选取我院2021年5月~2023年4月收治的97例血瘀湿浊证CRF患者,随机数字表法分为2组,对照组(n=48)使用腹膜透析治疗,观察组(n=49)予以益肾化瘀降浊方联合腹膜透析治疗。比较两组治疗6个疗程后的临床疗效;记录治疗前及治疗6个疗程后的中医证候积分、残余肾功能指标[血尿素氮(BUN)、血肌酐(SCr)、RRF]、炎性指标[白细胞介素6(IL-6)、高敏C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]水平差异;记录两组患者不良反应。结果:观察组总有效率显著高于对照组(P<0.05);两组患者中医证候积分、BUN、SCr、炎性指标(IL-6、CRP、TNF-α)水平显著降低,且观察组低于对照组(P<0.05);两组患者不良反应发生率差异无统计学意义(P>0.05)。结论:对CRF患者实施益肾化瘀降浊方联合PD治疗,不仅能有效缓解患者临床症状,改善疗效,还有助于恢复患者肾功能。

Objective To explore the influence of Yishen Huayu Jiangzhuo decoction combined with peritoneal dialysis(PD)on residual renal function(RRF)in patients with chronic renal failure(CRF)of blood stasis and dampness turbidity syndrome.Methods 97patients with CRF of blood stasis and dampness turbidity syndrome in our hospital from May 2021to April 2023were selected and divided into two groups by adopting the random number table method.The control group(n=48)was treated with PD,while the observation group(n=49)was given Yishen Huayu Jiangzhuo decoction combined with PD therapy.The clinical efficacy after 6courses of treatment was compared.The differences in TCM syndromes scores,residual renal function indicators[blood urea nitrogen(BUN),serum creatinine(SCr),RRF],and inflammatory indicators[interleukin-6(IL-6),high-sensitivity C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)]were recorded before treatment and after 6courses of treatment.The adverse reactions of the two groups were recorded.Results The total effective rate in observation group was significantly higher(P<0.05).The scores of TCM syndromes and levels of BUN,SCr,RRF,and inflammatory indicators(IL-6,CRP,TNF-α)were significantly decreased in both groups,and the above scores and levels were lower in observation group(P<0.05).There were no statistical differences in the incidence rates of adverse reactions between groups(P>0.05).Conclusion Yishen Huayu Jiangzhuo decoction combined with PD can not only effectively alleviate the clinical symptoms of patients and improve the efficacy,but also help to restore the renal function in CRF patients.
