摘要
目的分析阻力呼吸训练在老年肺癌患者术后的应用效果。方法选取2021年10月至2022年10月河南省胸科医院收治的110例老年肺癌患者,均接受手术治疗。运用随机数字表法对患者进行分组,分别是常规组、试验组,分别应用常规护理、阻力呼吸训练。对比两组康复状况、膈肌运动状况、肺功能[用力肺活量(FVC)、呼气峰速度(PEF)、6分钟步行测试(6MWT)]、Borg呼吸困难量表(Borg评分)、急性生理和慢性健康量表(APACHEⅡ)、并发症发生率。结果试验组住院天数、胸导管留置时间、呼吸机使用时间较常规组短(P<0.05);干预后试验组术侧、健侧膈肌运动状况较常规组高(P<0.05),FVC、6MWT、PEF水平较常规组高(P<0.05),APACHEⅡ、Borg评分较常规组低(P<0.05),并发症发生率较常规组低(P<0.05)。结论阻力呼吸训练可改善老年肺癌术患者的肺功能、膈肌运动,可减轻呼吸困难,降低并发症发生率,加快康复进程。
Objective To analyze the application effect of resistance breathing training in elderly patients with lung cancer after surgery.Methods A total of 110 eldly lung cancer patients admitted to Henan Provincial Chest Hospital form October 2021 to October 2022,all patients received surgical treatment.Patients were grouped using random number tables.In the conventional group and the experimental group,the routine nursing training and resistance breathing training were applied respectively.The rehabilitation status,diaphragm movement status,lung function[forced vital capacity(FVC),peak expiratory flow(PEF),6-minute walking test(6MWT)],Borg dyspnea scale(Borg scale),acute physiology and chronic health evaluationⅡ(APACHEⅡ),complication rate were compared between the two groups.Results The length of hospital stay,catheter retention time and ventilator use time in the experimental group were shorter than those in the conventional group(P<0.05).After intervention,the diaphragmatic muscle movement of the experimental group was higher than that of the conventional group(P<0.05).After intervention,the levels of FVC,6MWT and PEF in experimental group were higher than those in conventional group(P<0.05).After intervention,the scores of APACHEⅡand Borg in the experimental group were lower than those in the conventional group(P<0.05).The complication rate of the experimental group was lower than that of the conventional group(P<0.05).Conclusion Resistance breathing training can improve lung function and diaphragm movement in elderly lung cancer patients,reduce dyspnea,reduce the incidence of complications,and accelerate the rehabilitation process.
