心力衰竭早期预警配合运动康复训练在冠心病合并慢性心力衰竭中的应用效果观察

Application effect of early warning of heart failure combined with exercise rehabilitation training in coronary heart disease with chronic heart failure

目的探讨心衰早期预警配合运动康复训练在冠心病合并慢性心力衰竭中的应用效果。方法选取2021年5月至2023年1月我院收治的冠心病合并慢性心力衰竭患者104例,根据随机数表法分为试验组和参照组,参照组采用常规护理干预措施,试验组并给予心衰早期预警配合运动康复训练干预,比较两组患者干预前后心功能指标、6MWT和自我管理能力评分、上下肢FMA评分,并对两组患者并发症发生率进行比较。结果相比于参照组,干预后试验组患者LVEF更高,LVESD和LVEDD水平更低(P<0.05)。相比于参照组,干预后试验组患者6MWT和自我管理能力评分更高(P<0.05)。相比于参照组,干预后试验组患者上肢FMA评分和下肢FMA评分更高(P<0.05)。试验组患者并发症发生率(5.77%)明显低于参照组(21.15%)(χ^(2)=5.283,P=0.022)。结论心衰早期预警配合运动康复训练可提高冠心病合并慢性心力衰竭患者心功能,改善6分钟步行距离,提高自我管理能力,改善运动功能,减少并发症,值得进一步推广。

Objective To investigate the application effect of early warning of heart failure combined with exercise rehabilitation training in coronary heart disease(CHD)with chronic heart failure.Mehtods A total of 104 patients with CHD and chronic heart failure who were admitted to our hospital from May 2021 to January 2023 were enrolled and divided into experimental group and reference group using a random number table.The patients in the reference group were given routine nursing intervention,and those in the experimental group were given early warning of heart failure combined with exercise rehabilitation training.The two groups were compared in terms of cardiac function parameters,six-minute walk test(6MWT)score,self-management ability score,and Fugl-Meyer Assessment(FMA)scores of the upper and lower limbs before and after intervention,as well as the incidence rate of complications.Results Compared with the reference group,the experimental group had a significantly higher left ventricular ejection fraction and significantly lower left ventricular end-systolic diameter and left ventricular end-diastolic diameter after intervention(P<0.05).Compared with the reference group,the experimental group had significantly higher scores of 6MWT and self-management ability(P<0.05).Compared with the reference group,the experimental group had significantly higher FMA scores of the upper and lower limbs(P<0.05).The experimental group had a significantly lower incidence rate of complications than the reference group(5.77%vs 21.15%,χ^(2)=5.283,P=0.022).Conclusion For patients with CHD and chronic heart failure,early warning of heart failure combined with exercise rehabilitation training can improve cardiac function,6-minute walk distance,self-management ability,and motor function and reduce complications,and therefore,it holds promise for clinical application.
