桑菊止嗽散加减联合阿奇霉素序贯疗法治疗风热证小儿肺炎支原体感染临床研究

Clinical Study on Modified Sangju Zhisou Powder Combined with Azithromycin Sequential Therapy for Mycoplasma Pneumonia Infection in Children with Wind-Heat Syndrome

摘要 目的:观察桑菊止嗽散加减联合阿奇霉素序贯疗法治疗风热证小儿肺炎支原体(MP)感染的临床疗效。方法:按随机数字法将102例风热证MP感染患儿分为治疗组与对照组,每组51例。治疗组以桑菊止嗽散加减联合阿奇霉素序贯疗法治疗,对照组仅给予阿奇霉素序贯疗法治疗。治疗2周后,比较2组临床疗效、中医证候评分、症状缓解时间及血清炎症因子[干扰素-γ (IFN-γ)、C-反应蛋白(CRP)、肿瘤坏死因子-α (TNF-α)]水平。结果:治疗后,治疗组总有效率98.04%,高于对照组86.27%,2组比较,差异有统计学意义(P<0.05)。治疗组退热时间及咳嗽、咳痰、肺啰音消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,2组中医证候评分均较治疗前降低(P<0.05),且治疗组中医证候评分低于对照组(P<0.05)。治疗后,2组血清IFN-γ、CRP、TNF-α水平均较治疗前降低(P<0.05),且治疗组3项指标水平均低于对照组(P<0.05)。结论:桑菊止嗽散加减联合阿奇霉素序贯疗法治疗风热证MP感染疗效显著,能有效减轻机体炎症反应,改善中医证候,加速病情康复。 Objective:To observe the clinical effect of modified Sangju Zhisou Powder combined with Azithromycin sequential therapy on mycoplasma pneumonia(MP)infection in children with wind-heat syndrome.Methods:A total of 102 children with MP infection with wind-heat syndrome were divided into the treatment group and the control group according to the radom rumber table method,with 51 cases in each group.The treatment group was treated with modified Sangju Zhisou Powder combined with Azithromycin sequential therapy,and the control group was only treated with Azithromycin sequential therapy.After two weeks of treatment,the clinical effects,traditional Chinese medicine(TCM)syndrome scores,remission time of symptoms and levels of serum inflammatory factors[interferon-γ(IFN-γ),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)]were compared between the two groups.Results:After treatment,the total effective rate was 98.04%in the treatment group,higher than that of 86.27%in the control group,the difference being significant(P<0.05).The disappearance time of fever,cough,expectoration and rales in the treatment group was respectively shorter than that in the control group,the difference being significant(P<0.05).After treatment,the TCM syndrome scores in the two groups were reduced when compared with those before treatment(P<0.05),and the TCM syndrome score in the treatment group was lower than that in the control group(P<0.05).After treatment,the levels of serum IFN-γ,CRP and TNF-αin the two groups were declined when compared with those before treatment(P<0.05),and the above three levels in the treatment group were lower than those in the control group(P<0.05).Conclusion:The therapy of modified Sangju Zhisou Powder combined with Azithromycin sequential therapy has a significant curative effect on MP infection with wind-heat syndrome,which can effectively reduce the inflammatory responses of the body,improve the TCM syndrome and accelerate the recovery.
