复方斑蝥胶囊联合紫杉醇、卡铂治疗晚期非小细胞肺癌疗效观察及对血清NRP-1、B7-H3水平的影响

Effect of Compound Cantharides Capsules Combined with Paclitaxel and Carboplatin on Advanced Non-Small Cell Lung Cancer and Its Influence on Serum NRP-1 and B7-H3 Levels

目的:观察复方斑蝥胶囊联合紫杉醇、卡铂治疗晚期非小细胞肺癌的临床疗效及对血清神经纤毛蛋白1 (NRP-1)、B7-H3水平的影响。方法:选取80例晚期非小细胞肺癌患者,按照随机数字表法分为对照组和研究组各40例。对照组给予紫杉醇加卡铂化疗方案,研究组在对照组基础上加用复方斑蝥胶囊治疗。比较2组临床疗效,简易智能精神状态检查量表(MMSE)、日常生活能力量表(ADL)评分,CD3^(+)、CD4^(+)和自然杀伤(NK)细胞以及血清NRP-1、B7-H3水平。结果:研究组总有效率为75.00%,对照组为40.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组MMSE、ADL评分均较治疗前升高(P<0.05),且研究组2项评分均高于对照组(P<0.05)。治疗后,2组CD4^(+)、NK细胞水平均较治疗前升高(P<0.05),CD8^(+)水平较治疗前降低(P<0.05);且研究组CD4^(+)、NK细胞水平高于对照组(P<0.05),CD8^(+)水平低于对照组(P<0.05)。治疗后,2组血清NRP-1、B7-H3水平均较治疗前降低(P<0.05),且研究组2项水平均低于对照组(P<0.05)。结论:复方斑蝥胶囊联合紫杉醇加卡铂化疗治疗晚期非小细胞肺癌疗效显著,可有效改善患者免疫功能,下调血清NRP-1、B7-H3水平,提高患者认知功能与生活能力。

Objective:To observe the clinical effect of Compound Cantharides Capsules combined with Paclitaxel and Carboplatin on advanced non-small cell lung cancer and its influence on serum neuropilin-1(NRP-1)and B7-H3 levels.Methods:Eighty patients with advanced non-small cell lung cancer were selected and divided into the control group and the study group according to the random number table method,with 40 cases in each group.The control group was given chemotherapy of Paclitaxel and Carboplatin,and the study group was additionally treated with Compound Cantharides Capsules based on the treatment of the control group.The clinical effects,Mini-Mental State Examination(MMSE)and Activities of Daily Living(ADL),CD3^(+),CD4^(+)and natural killer(NK)cells,and serum NRP-1 and B7-H3 levels in the two groups were compared.Results:The total effective rate was 75.00%in the study group,and 40.00%in the control group,the intergroup comparison showed that the difference being significant(P<0.05).After treatment,the scores of MMSE and ADL in the two groups were increased when compared with those before treatment(P<0.05),and the above two scores in the study group were higher than those in the control group(P<0.05).After treatment,the levels of CD4^(+)and NK cells in the two groups were increased when compared with those before treatment(P<0.05),and the CD8^(+)levels were decreased when compared with those before treatment(P<0.05);the levels of CD4^(+)and NK cells in the study group were higher than those in the control group(P<0.05),and the CD8^(+)level was lower than that in the control group(P<0.05).After treatment,the levels of serum NRP-1 and B7-H3 in the two groups were decreased when compared with those before treatment(P<0.05),and the above two levels in the study group were lower than those in the control group(P<0.05).Conclusion:The therapy of Compound Cantharides Capsules combined with chemotherapy of Paclitaxel and Carboplatin has a significant curative effect on advanced non-small cell lung cancer,which can effectively improve the immune function of patients,down-regulate the levels of serum NRP-1 and B7-H3,and improve the cognitive function and activities of daily living.
