浅析吴尚先痹证外治的诊疗特色

Brief Analysis on Characteristics of WU Shangxian’s Diagnosis and Treatment of Arthralgia Syndrome

[目的]探析吴尚先《理瀹骈文》中外治疗痹证的学术思想,以期为临床应用外治法治疗痹证提供指导。[方法]通过研读《理瀹骈文》中痹证相关内容,并对吴尚先治疗痹证的方药加以理解分析,从用药特点和外治方法选用两个方面总结诊疗特色,归纳学术思想。[结果]吴氏在前人基础上提出“外治之理即内治之理,外治之药即内治之药”的理论,其痹证用药特点主要有三个方面:药用生猛,辛香走窜;辛散行气,广略取胜;循经用药,辨证治疗。另外,根据不同临床表现或证候,灵活选用不同外治法,如痛处不移、痛剧用贴法、包法,寒痹用熨法,行痹用抹法涂法。[结论]吴尚先丰富了痹证中医外治法,为现代临床痹证的治疗提供了新的思路。

[Objective Objective]To explore the academic thoughts on external treatments for paralysis syndrome in WU Shangxian’s Liyue Pianwen,in order to provide guidance for the clinical application of external treatment methods in the treatment of paralysis syndrome.[Methods Methods]By studying the relevant contents of arthralgia syndrome in Liyue Pianwen,and understanding and analyzing WU Shangxian’s prescriptions for treating arthralgia syndrome,this paper summarizes the characteristics of diagnosis and treatment from two aspects:the characteristics of medication and the selection of external treatment methods,and sums up academic thoughts.[Results Results]On the basis of his predecessors,WU put forward the theory that“the principle of external treatment was just the theory of internal treatment,and the medicine of external treatment was the medicine of internal treatment”.There were three main characteristics of the use of drugs in arthralgia syndrome:strong medicinal use and spicy fragrance;using spicy drugs with a dispersing effect to smooth Qi,and using a variety of methods for treatment together;medication along meridians and syndrome differentiation treatment.In addition,according to different clinical manifestations or syndromes,such as pain place did not move,pain drama,with sticking method,package method;cold arthralgia,with ironing method;arthralgia,with plaster method.[Conclusion Conclusion]WU Shangxian enriched the external treatment of arthralgia syndrome in traditional Chinese medicine and provided a new idea for the treatment of arthralgia syndrome in modern clinic.
