肾素-血管紧张素系统在瘢痕疙瘩中的作用研究进展

摘要 瘢痕疙瘩是一种纤维增生性疾病,其特征是皮肤损伤愈合过程中,过度炎症导致细胞外基质成分沉积过多。目前瘢痕疙瘩的发病机制尚未得到完全阐明,治疗策略相对有限。肾素-血管紧张素系统(RAS)的激活与多种器官的纤维化有关。RAS是一个激素系统,其中血管紧张素Ⅱ(AngⅡ)是主要激素,而AngⅡ的主要受体为血管紧张素受体1(AT1R)和血管紧张素受体2(AT2R)。现有研究表明RAS不仅存在于表皮和真皮层,也存在于皮下脂肪组织、微血管和附属物如毛囊、皮脂腺和汗腺中。本文综述了RAS在伤口愈合和瘢痕形成中的作用,并讨论了靶向调控RAS系统治疗和预防瘢痕疙瘩的转化潜力。 Keloid is a fibroproliferative disease characterized by excessive deposition of extracellular matrix components resulting from hyperinflammation during the healing process of skin injuries.Currently,the pathogenesis of keloids remains incompletely understood,and treatment strategies are relatively limited.The activation of the renin-angiotensin system(RAS)has been associated with fibrosis in various organs.RAS is a hormonal system,in which angiotensin II(AngII)is the primary hormone,and the main receptors of Ang II are angiotensin receptor 1(AT1R)and angiotensin receptor 2(AT2R).Existing research indicates that RAS is present not only in the epidermis and dermis,but also in subcutaneous adipose tissue,microvasculature and appendages such as hair follicles,sebaceous glands and sweat glands.This article reviews the role of RAS in wound healing and scar formation,and discusses the translational potential of targeted modulation of the RAS system in the treatment and prevention of keloids.
