病情分级预警护理在CCU急性心肌梗死患者病情改善中的应用效果分析

Effect of grading early warning nursing on improving the condition of patients with acute myocardial infarction in CCU

目的 探讨心脏重症监护室(CCU)急性心肌梗死患者应用病情分级预警护理对病情改善的效果。方法 选取120例自2020年3月—2023年3月在淮安市第一人民医院CCU治疗的急性心肌梗死患者120例,以随机数字表法分为对照组(n=60,行常规CCU护理)与试验组(n=60,在对照组基础上实施病情分级预警护理),比较两组患者心脏相关并发症发生情况;护理前、护理后心率相关指标水平、各项心脏功能指标水平;总住院时间。结果 试验组心律失常、心源性休克、心包填塞、梗死再发生总并发症发生率低于对照组,差异有统计学意义(P<0.05);护理后,试验组心电图校正后QT(QTC)间期高于对照组,QT间期离散度(QTd)、心率低于对照组,差异有统计学意义(P<0.05);试验组急性生理评分、慢性生理评分及APACHEⅡ系统总分低于对照组,差异有统计学意义(P<0.05);试验组总住院时间少于对照组,差异有统计学意义(P<0.05)。结论 病情分级预警护理有助于降低CCU急性心肌梗死患者心脏相关并发症的发生,促进心率及病情状态的改善,缩短总住院时间。

Objective To study the effect of grading early warning nursing for patients with acute myocardial infarction(AMI)in the Coronary Care Unit(CCU).Methods One hundred and twenty patients with AMI who were admitted to CCU in Huai'an First People's Hospital from March 2020 to March 2023 conducted on this study.All patients were randomly divided into control group(routine CCU nursing,n=60)and experimental group(early warning nursing of disease classification on the basis of control group,n=60).The incidence of heart-related complications;before and after nursing,the levels of heart rate related indexes and various cardiac function indexes;total hospitalization time were compared between two groups.Results The incidence of arrhythmia,cardiogenic shock,cardiac tamponade and recurrence of infarction in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05).There were significant differences in QTC(corrected QT interval,QTC);QT interval dispersion(QTd)and heart rate between the two groups(P<0.05).The acute physiological score,chronic physiological score and APACHEⅡscore of the experimental group were lower than those of the control group(P<0.05).There was a significant difference in the total hospitalization time between the two groups(P<0.05).Conclusion Grading early warning nursing can help reduce the occurrence of heart-related complications and shorten the total hospitalization time in CCU patients with AMI.
