首发青少年抑郁症患者细胞炎性因子的相关研究

Correlation study of cytokines in first-episode adolescent depression

目的:探讨首发青少年抑郁症患者细胞炎症因子水平的变化以及与临床症状之间的相关性。方法:纳入2022年8月至11月于滨州医学院附属医院心理科就诊的首发青少年抑郁症患者42例和健康青少年30名,应用一般问卷、汉密尔顿抑郁量表17项(Hamilton depression scale 17-item,HAMD-17)、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)、抑郁自评量表(self-rating depression scale,SDS)、焦虑自评量表(self-rating anxiety scale,SAS)、症状自评量表(self-rating symptom scale,SCL-90)、青少年生活事件问卷(adolescent life events questionnaire,ASLEC)评定患者的临床症状,采用ELISA法检测抑郁症患者和对照组的细胞炎性因子和皮质醇激素水平。结果:有自杀意念患者细胞炎性因子TNF-α浓度高于无自杀意念患者,差异有统计学意义(t=2.219,P=0.034);TNF-α浓度水平与SDS、SAS、SCL-90评分之间存在正相关,有统计学意义(r=0.465、0.374、0.369,P均<0.05);并与ASLEC量表评分中学习压力因子、受惩罚因子之间存在正相关,有统计学意义(r=0.337、0.315,P均<0.05)。结论:细胞炎性因子TNF-α浓度在有无自杀意念患者中存在差异,可能作为青少年抑郁症患者自杀意念、主观情绪症状及学习压力的生物学预测因素。

Objective:To explore the changes of inflammatory cytokines and its relationship with clinical symptoms in the first-episode adolescent depression.Method:A total of 42 first-episode adolescents with depression treated in the Psychology Department of Binzhou Medical University Hospital from August to November 2022 and 30 healthy adolescents were enrolled in the study,the clinical symptoms of the patients were assessed by general questionnaire,Hamilton depression scale 17-item(HAMD-17),Hamilton anxiety scale(HAMA),self-rating depression scale(SDS),self-rating anxiety scale(SAS),self-rating symptom scale(SCL-90)and adolescent life events questionnaire(ASLEC).ELISA was used to detect the levels of inflammatory cytokines and cortisol in depressed patients and controls.Results:The level of TNF-αin patients with suicidal ideation was higher than that in patients without suicidal ideation,and the differences were statistically significant(t=2.219,P=0.034).The level of TNF-αwas positively correlated with the scores of SDS,SAS and SCL-90(r=0.465、0.374、0.369,All P<0.05).The level of TNF-αwas positively correlated with the learning stress factor and punishment factor of ASLEC scale scores(r=0.465、0.374、0.369,All P<0.05).Conclusion:The level of TNF-αis different in patients with or without suicidal ideation.TNF-αmay be a biological predictor of suicidal ideation,subjective symptoms and learning stress.in adolescents with depression.
