立位CT性能评估及在骨关节退行性疾病诊断应用

Performance evaluation of upright CT and its application in the diagnosis of degenerative bone and joint diseases

摘要 目的评估立式扫描X线计算机体层摄影设备(立位CT)的性能,包括成像能力、扫描流程及图像质量,并初步探究其在骨关节退行性疾病中的应用价值。方法使用与立位CT具有同等硬件配置的卧位扫描设备进行体模扫描并对比,评估成像能力。经本院伦理委员会审批后招募35名志愿者进行立位扫描,其中平扫29名,增强扫描6名,每名志愿者扫描一个部位,记录摆位时间、扫描时间、扫描剂量等参数,并与卧位扫描设备进行对比,评估扫描流程合理性。获得影像资料后,采用李克特(Likert)五分量表法评分,采用双人盲态评价的方式,评估图像质量。获得其中2名志愿者的前期卧位影像资料,对比卧位、立位差异。结果立位CT的成像能力与同等配置卧位CT相当、扫描流程合理、图像质量良好。通过图像对比发现,腰椎间盘突出症、膝关节骨关节炎患者卧位、立位影像存在显著差异。结论立位CT成像能力良好、扫描流程合理、图像清晰。立位CT对骨关节退行性疾病的诊断具有重要意义。 Objective To evaluate the performance of upright X-ray computed tomography equipment including imaging capability,workflow and image quality,and to preliminarily explore its application value in degenerative bone and joint diseases.Methods Use a supine scanning device with the same hardware confguration as upright CT to perform a comparative evaluation of the imaging ability through phantom scanning.After approval by the Ethics Committee of our hospital,35 volunteers were recruited to perform upright scans,including 29 plain scans and 6 enhanced scans.Each volunteer was scanned for a certain area,recording parameters such as positioning time,scanning time and scanning dose.The data were compared with supine position scanning equipment to evaluate the rationality of the work process.After obtaining the imaging data,the image quality was evaluated using the Likert five component scale by a two person blind evaluation method.Obtain supine CT image data of two volunteers and compare the differences between supine and upright positions.Results The imaging ability of upright CT was equivalent to that of supine CT with the same configuration.The scanning process was reasonable,and the image quality was good.Through image comparison,signifcant differences were found in supine and upright images between patients with lumbar disc herniation and knee osteoarthritis.Conclusions Upright CT has good imaging ability,reasonable workfow,and clear images.It will be of great signifcance in the diagnosis of degenerative diseases of bones and joints.
