视黄醇结合蛋白测定在新生儿补充维生素A中的应用研究

摘要 目的:探究视黄醇结合蛋白(Retinol binding protein,RBP)测定在新生儿补充维生素A中的应用效果。方法:遴选2019.6至2023.12入住我院新生儿、早产儿病房的新生儿纳入研究,依照胎龄不同将28~32 w分为A组,33~36 w为B组,37~42 w为C组,每组均40例,入院后每日给予维生素A。对比不同胎龄儿在补充维生素A前RBP测定值,分析RBP水平与胎龄关系;三组胎儿补充维生素A前后,采用免疫比浊法测定RBP水平变化情况,分析其与补充维生素A的关系。结果:不同胎龄胎儿RBP水平与胎龄呈正相关(r=0.224,P<0.05);治疗前,三组胎儿RBP水平比较有统计学意义(P<0.05);治疗后,三组胎儿RBP水平均显著升高(P<0.05),且A组胎儿RBP水平变化显著高于B组和C组(P<0.05);RBP水平与补充维生素A呈正相关(r=0.286,P<0.05)。结论:RBP水平与胎龄有一定相关性,RBP测定有助于确定新生儿维生素A水平,可用于临床补充维生素A疗效评价标准,且胎龄小,补充维生素A的效果较好。 Objective:To explore the application effect of retinol binding protein(RBP)determination in neonatal vitamin A supplementation.Methods:Neonates in neonatal and premature infant wards of our hospital were selected from June 2019 to December 2023 were retrospectively selected.According to different gestational ages,neonates of 28-32 weeks were set as group A,neonates of 33-36 weeks were included in group B,and neonates of 37-42 weeks were enrolled as group C,with 40 cases in each group.Vitamin A was given daily after admission.The RBP measurement value was compared among infants with different gestational ages before vitamin A supplementation,and the relationship between RBP level and gestational age was analyzed.The change of RBP level detected by immunoturbidimetry before and after vitamin A supplementation was compared among the three groups,and the relationship with vitamin A supplementation was analyzed.Results:The level of RBP was positively correlated with gestational age(r=0.224,P<0.05).Before treatment,the level of fetal RBP in the three groups was statistically significant(P<0.05).After treatment,the level of fetal RBP in the three groups was significantly increased(P<0.05),and the change of fetal RBP level in group A was significantly higher than that in group B and group C(P<0.05).RBP level was positively correlated with vitamin A supplementation(r=0.286,P<0.05).Conclusion:There is a certain correlation between RBP level and gestational age.The determination of RBP is helpful to determine the level of vitamin A in neonates,and it can be used as the evaluation standard of clinical efficacy of vitamin A supplementation.Moreover,with a young gestational age,supplementing with vitamin A has a better effect.
