动态监测血清FDP、S100β、FIB水平在脑梗死患者疗程中的变化及临床意义

摘要 目的:探究动态监测血清纤维蛋白降解产物(Fibrin degradation product,FDP)、中枢神经特异性蛋白β(Recombinant S100 calcium binding proteinβ,S100β)、纤维蛋白原(Fibrinogen,FIB)水平在脑梗死患者疗程中的变化及临床意义。方法:选取2021年9月至2023年9月期间本院收治的62例脑梗死患者作为研究对象。根据治疗效果分为显效组(19例)、有效组(29例)和无效组(14例)。分析对比患者治疗前、治疗7 d、14 d后血清FDP、S100β、FIB水平。分析对比不同时间点三组血清FDP、S100β、FIB水平并分析其与治疗效果的相关性。分析治疗7 d后血清FDP、S100β、FIB水平对脑梗死患者治疗无效的预测价值。结果:治疗14 d后,血清FDP、S100β、FIB水平<治疗7 d后<治疗前(P<0.05)。不同时间点治疗无效组的血清FDP、S100β、FIB水平均显著高于显效组、有效组;治疗有效组的血清FDP、S100β、FIB水平显著高于显效组(P<0.05)。治疗14 d后,血清FDP、S100β、FIB水平与脑梗死患者治疗效果呈正相关(P<0.05)。治疗7 d后血清FDP、S100β、FIB水平联合预测脑梗死治疗无效的曲线下面积(area under the curve,AUC)为0.896,显著高于单一指标的检测(P<0.05)。结论:血清FDP、S100β、FIB水平在脑梗死患者疗程中变化显著,可为临床预测脑梗死患者治疗无效提供一定参考价值。 Objective:To explore the changes and clinical significance of dynamic monitoring of serum fibrin degradation product(FDP),central nervous specific proteinβ(S100β)and fibrinogen(FIB)levels in patients with cerebral infarction during treatment course.Methods:62 patients with cerebral infarction treated in our hospital from September 2021 to September 2023 were selected as the study objects.According to the treatment effect,they were divided into obvious group(19 cases),effective group(29 cases)and ineffective group(14 cases).The serum levels of FDP,S100βand FIB were analyzed and compared before treatment,7 days and 14 days after treatment.The levels of serum FDP,S100βand FIB in three groups at different time points were analyzed and compared,and the correlation between them and the therapeutic effect was analyzed.To analyze the predictive value of serum FDP,S100βand FIB levels in patients with cerebral infarction after 7 days of treatment.Results:The serum FDP,S100βand FIB levels were lower after 14 days of treatment than after 7 days of treatment than before treatment(P<0.05).The levels of serum FDP,S100βand FIB in the ineffective group were significantly higher than those in the obvious and effective groups at different time points.The levels of serum FDP,S100βand FIB in the effective treatment group were significantly higher than those in the effective treatment group(P<0.05).After 14 days of treatment,serum FDP,S100βand FIB levels were positively correlated with the therapeutic effect of cerebral infarction patients(P<0.05).After 7 days of treatment,the AUC of serum FDP,S100βand FIB combined to predict the ineffectiveness of cerebral infarction was 0.896,which was significantly higher than that of single index(P<0.05).Conclusion:Serum FDP,S100βand FIB levels change significantly during the course of treatment in patients with cerebral infarction,which can provide a certain reference value for clinical prediction of treatment ineffectiveness in patients with cerebral infarction.
