重组牛碱性成纤维细胞生长因子眼用凝胶联合妥布霉素地塞米松滴眼液对白内障术后干眼症患者泪膜破裂时间的影响

Effect of Recombinant Bovine Alkaline Fibroblast Growth Factor Eye Gel Combined with Tobramycin Dexamethasone Eye Drops on Tear Film Rup⁃ture Time in Dry Eye Patients after Cataract Surgery

目的分析白内障术后干眼症给予重组牛碱性成纤维细胞生长因子眼用凝胶联合妥布霉素地塞米松滴眼液治疗的效果,并探讨对患者泪膜破裂时间的影响。方法目的选取南京医科大学眼科医院2021年1月—2023年6月收治的白内障术后干眼症患者84例,按治疗方法不同分组,两组均42例,参照组给予重组牛碱性成纤维细胞生长因子眼用凝胶治疗,研究组加用妥布霉素地塞米松滴眼液治疗。比较两组治疗前、治疗1个月后的眼部症状、泪膜稳定性、炎症因子水平,并统计两组的不良反应发生率。结果治疗后,研究组眼表疾病指数评分(18.17±5.52)分、角膜荧光染色评分(0.77±0.27)分低于参照组(24.73±6.62)分、(1.38±0.25)分,差异有统计学意义(t=4.932、10.744,P均<0.05)。治疗后,研究组泪膜稳定性指标均优于参照组,差异有统计学意义(P均<0.05)。治疗后,研究组血清白细胞介素-1β、白细胞介素-6、肿瘤坏死因子-α水平低于参照组,差异有统计学意义(P均<0.05)。两组红肿、结膜充血、瘙痒、头痛不良反应总发生率比较,差异无统计学意义(P均>0.05)。结论采用重组牛碱性成纤维细胞生长因子眼用凝胶联合妥布霉素地塞米松滴眼液治疗白内障术后干眼症患者,可改善其泪膜破裂时间,减轻干眼症状,缓解炎症,且具有一定的安全性。

Objective To analyze the effect of recombinant bovine alkaline fibroblast growth factor eye gel combined with tobramycin dexamethasone eye drops on tear film rupture time in patients with dry eye after cataract surgery.Methods A total of 84 cases with dry eye patients after cataract surgery in Nanjing Medical University Eye Hospital from January 2021 to June 2023 were objectively selected,they were divided into two groups by difference treatment,with 42 cases,the reference group received recombinant bovine alkaline fibroblast growth factor eye gel treatment,the study group added tobramycin dexamethasone eye drops treatment.Compare the ocular symptoms,tear film stability,and inflammatory factor levels before and 1 month after treatment,and calculate the incidence of adverse effects in the two groups.Results After treatment,the scores of OSFI and CFS were 18.17±5.52 and 0.77±0.27 in the study group,lower than 24.73±6.62,and 1.38±0.25 in the control group,and the difference was statistically significant(t=4.932,10.744,both P<0.05).After treatment,the tear film stability indexes in the study group were better than those in the reference group,the differences were statistically significant(both P<0.05).After treatment,the serum levels of inter-leukin-1β,interleukin-6,and TNF-αin the study group were lower than those in the reference group,the differ-ences were statistically significant(all P<0.05).There was no statistically significant difference in the overall inci-dence of redness,conjunctival congestion,pruritus,and headache between the two groups(P>0.05).Conclusion The patients with dry eye after cataract operation were treated with recombinant bovine alkaline fibroblast growth factor eye gel combined with tobramycin dexamethasone eye drops with cataract can improve the tear film rupture time,alle-viate dry eye symptoms,relieve inflammation,and have certain safety.
