动态增强CT扫描与磁共振成像对肝细胞癌的诊断效能

Diagnostic Efficacy of Dynamic Enhanced CT Scanning and Magnetic Resonance Imaging in Hepatocellular Carcinoma

摘要 目的:探讨在肝细胞癌诊断中应用动态增强CT扫描与磁共振成像(MRI)的临床效果.方法:选取以2020年3月至2023年3月本院收治的98例肝脏病变患者为研究对象.均开展动态增强CT扫描与MRI检查,以手术病理结果为金标准,比较这两种检查方式对肝细胞癌的诊断结果、与金标准结果的一致性强度、诊断效能.结果:98例肝癌病变患者,手术病理诊断恶性为50例,良性为48例,MRI诊断结果与手术病理结果之间一致性强度(Kappa=0.8364)及其诊断肝细胞癌的敏感性(96.00%)、特异性(87.50%)和准确性(91.84%)均明显高于动态增强CT扫描诊断(Kappa=0.4471、80.00%、64.58%、72.54%),差异均有统计学意义(P<0.05).结论:在肝细胞癌诊断中应用动态增强CT扫描与MRI检查,后者诊断效果明显高于前者,不仅具有更高诊断准确性,其敏感性和特异性也更高. Objective:To investigate the clinical effect of dynamic enhanced CT scanning and magnetic resonance imaging(MRI)in the diagnosis of hepatocellular carcinoma.Methods:A total of 98 patients with liver disease admitted to our hospital from March 2020 to March 2023 were selected as the study objects,and dynamic enhanced CT scan and MRI were carried out in all of them,with surgical pathological results as the gold standard.The diagnostic results of hepatocellular carcinoma,the consistency and diagnostic efficacy of these two examination methods were compared with the gold standard results.Results:In 98 patients with liver cancer,50 cases were malignant and 48 cases were benign.The consistency between MRI diagnosis and pathological results(Kappa=0.8364)and the sensitivity(96.00%),specificity(87.50%)and accuracy(91.84%)of HCC diagnosis were significantly higher than those of dynamic enhanced CT diagnosis(Kappa=0.4471,80.00%,64.58%,72.54%),and the differences were statistically significant(P<0.05).Conclusion:In the diagnosis of hepatocellular carcinoma using dynamic enhanced CT scan and MRI,the latter is significantly more effective than the former,not only has higher diagnostic accuracy,but also has higher sensitivity and specificity.
