基于数据挖掘探究李学军教授诊治慢性萎缩性胃炎用药规律

Exploring the medication rules of Professor Li Xuejun in the diagnosis and treatment of chronic atrophic gastritis based on data mining

目的中医药治疗慢性萎缩性胃炎(CAG)具有一定优势,本研究将李学军教授治疗CAG患者的用药规律通过数据挖掘技术进行探索分析,同时进一步总结其临床经验。方法采集李教授门诊(安徽中医药大学第二附属医院)首诊的2022年1—12月的CAG患者的医案,将病案数据通过SPSS Statistics 26.0、SPSS Modeler 18.0和Excel 2010软件进行关联规则分析、频数统计及聚类分析。结果本研究纳入CAG患者医案共141例,其中共有中药112味,核心药物为柴胡、半夏、莪术、白花蛇舌草等,聚类分析得出核心处方及基础方。结论李教授认为CAG最常见证型是脾胃虚弱型,CAG中医诊断以胃痞为多,柴胡、半夏、莪术、茯苓、薏苡仁、甘草、白花蛇舌草为李教授治疗CAG的常用药物,在药性方面以寒性药及温性药为主,在药味方面多用苦味、甘味和辛味药物,李教授治疗CAG的基础方是通过聚类分析而得出的四组核心处方中的第三组方。

Objective Traditional Chinese medicine has certain advantages in treating chronic atrophic gastritis(CAG).This study uses data mining technology to explore and analyze the medication rules of CAG patients treated by Professor Li Xuejun,and summarizes their clinical experience.Methods The first-time medical records of CAG patients diagnosed and treated by Professor Li Xuejun from the Second Affiliated Hospital of Anhui University of Chinese Medicine from January to December 2022 were collected,and the data were analyz ed by SPSS Statistics 26.0,SPSS Modeler 18.0 and Excel 2010.Results A total of 141 medical records of CAG patients were included,including 112 kinds of traditional Chinese medicines.The core medicines were radix bupleuri,pinellia ternata,curcuma zedoary,hedyotis diffusa,etc.The core prescription and basic prescription were obtained by cluster analysis.Conclusion Professor Li Xuejun believes that spleen and stomach weakness type is the most common syndrome of CAG,and the majority of CAG is stomach distension.Bupleurum,Banxia,Curcuma,Poria cocos,Coix seed,licorice,and white-flowered snake tongue are commonly used drugs by Professor Li to treat CAG.In terms of drug properties,cold and warm drugs are mainly used,while bitter,spicy,and sweet Chinese medicine are mainly used to treat CAG.The third group of core prescriptions identified through cluster analysis is Professor Li Xuejun’s basic prescription for treating CAG.
