阴道镜联合LEEP术在子宫颈癌前病变患者中的临床价值研究

Clinical Value of Colposcopy Combined with LEEP Surgery in Patients with Precancerous Lesions of the Cervix

目的 探究阴道镜联合宫颈环形电切术(Loop Electrosurgical Excision Procedure, LEEP)对子宫颈癌前病变的临床价值。方法 方便选取2022年1月—2023年9月睢宁县妇幼保健院收治的103例子宫颈癌前病变患者为研究对象,采用密闭信封法分为对照组(n=51)、研究组(n=52)。对照组行宫颈微波治疗,研究组行阴道镜联合LEEP术治疗。对比两组患者手术指标,并发症发生率,术前及术后3 d炎症因子,术前及术后3个月生活质量。结果 研究组手术时间(22.63±3.52)min,术中出血量(16.83±2.78)mL,住院时间(4.25±0.91)d,均优于对照组,差异有统计学意义(t=17.059、12.119、8.432,P均<0.001)。术前,两组患者炎症因子水平对比,差异无统计学意义(P>0.05);术后3 d,研究组炎症因子水平均低于对照组,差异有统计学意义(P<0.001)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。术前,两组患者生活质量评分对比,差异无统计学意义(P>0.05);术后3个月,研究组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论 子宫颈癌前病变以阴道镜联合LEEP术治疗,可改善手术指标,控制炎症反应,加快患者术后恢复,从而提高患者生活质量,具有较高安全性。

Objective To explore the clinical value of colposcopy combined with Loop Electroresection Procedure(LEEP)in patients with precancerous lesions of the cervix.Methods A total of 103 patients with cervical precancerous lesions admitted to Suining Maternal and Child Health Hospital from January 2022 to September 2023 were conve-niently selected as the study objects.They were divided into control group(n=51)and study group(n=52)by using sealed envelope method.The control group received cervical microwave treatment,while the study group received col-poscopy combined with LEEP surgery.Surgical indexes,complication rate,inflammatory factors before and 3 days af-ter surgery,quality of life before and 3 months after surgery were compared between the two groups.Results The op-eration time of the study group was(22.63±3.52)min,the amount of intraoperative blood loss was(16.83±2.78)mL,and the length of hospital stay was(4.25±0.91)d,which were better than those of the control group,the differences were statistically significant(t=17.059,12.119,8.432,all P<0.001).Before operation,there was no statistically signifi-cant difference in the levels of various inflammatory factors between the two groups(P>0.05).3 days after surgery,the level of inflammatory factor in the study group was lower than that in the control group,and the difference was statisti-cally significant(P<0.001).The complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Before surgery,there was no significant difference in quality of life score between the two groups(P>0.05).Three months after operation,the quality of life score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Colposcopy combined with LEEP in the treatment of cervical precancerous lesions can improve surgical indexes,control inflamma-tory response,accelerate postoperative recovery,improve patients'quality of life,and have high safety.
