摘要
目的探讨体外冲击波(Extracorporeal Shock Wave Therapy,ESWT)治疗对膝骨性关节炎(Knee Osteoarthritis,KOA)患者疼痛、膝关节功能和日常生活活动能力的近期疗效。方法回顾性选取2020年7月—2023年7月察哈尔右翼后旗人民医院收治的的109例KOA患者的临床资料,根据治疗方法的不同将患者分为对照组(46例)和观察组(63例)。对照组患者采用运动治疗,观察组患者在运动治疗基础上联合ESWT,两组患者均给予常规治疗。比较两组患者治疗前后疼痛视觉模拟量表(Visual Analogue Scales,VAS)、膝主动关节活动度(Active Range of Motion,aROM)和西安大略和麦克马斯特大学骨关节炎指数(Western Ontario and McMaster Universities,WOMAC)评分。结果治疗后,两组患者VAS评分、WOMAC总分及子项分均低于治疗前,aROM高于治疗前,差异有统计学意义(P均<0.001)。观察组患者VAS评分WOMAC总分及疼痛、日常生活难度子项分均低于对照组,差异有统计学意义(P均<0.001)。观察组患者aROM高于对照组,差异有统计学意义(P<0.05)。两组患者WOMAC僵硬子项分对比,差异无统计学意义(P>0.05)。结论ESWT可减轻KOA患者疼痛,改善膝关节功能和日常生活能力,与运动疗法相辅相成。
Objective To investigate the short-term efficacy of Extracorporeal Shock Wave Therapy(ESWT)on pain,knee function and activities of daily living in patients with knee osteoarthritis(KOA).Methods The dinial data of 109 KOA patients who were treatmed in the People's Hospital of Chahar Right Wing Rear Banner from July 2020 to July 2023 were retrospectively selected.The patients were divided into control group(46 cases)and observation group(63 cases)by different treatment method.The patients in the control group were treated with exercise therapy,and the patients in the observation group were treated with ESWT on the basis of exercise therapy.Both groups were given routine treatment.The Pain Visual Analogue Scale(VAS)score,Knee Active Range of Motion(aROM)and Western Ontario and McMaster University Osteoarthritis Index(WOMAC)score were compared between patients in two groups before and after treatment.Results After treatment,the VAS score,WOMAC total score and sub-item score of the two groups were lower than those before treatment,and the aROM score was higher than that before treatment,and the differences were statistically significant(all P<0.001).The VAS score WOMAC total score and pain,daily life difficulty sub-item scores of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.001).The aROM of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in WOMAC stiffness sub-item scores between the two groups(P>0.05).Conclusion ESWT can relieve pain,improve knee joint function and daily living ability in patients with KOA,which is complementary to exercise therapy.
