临床药师主导的远程慢性病药学服务在老年高血压患者中的应用效果

Application Effect of Clinical Pharmacist-led Remote Pharmaceutical Care for Chronic Diseases in Elderly Patients with Hypertension

目的:探讨临床药师主导的远程慢性病药学服务在老年高血压患者中的应用效果。方法:选取2022年1月—2023年3月徐州医科大学附属医院收治的老年高血压患者210例为研究对象,随机分为对照组与观察组,各105例。对照组实施常规慢性病管理,观察组实施临床药师主导的远程慢性病药学服务。比较两组血压水平、服药依从性、药物知晓程度及不良反应发生率。结果:干预后,两组收缩压、舒张压水平降低,观察组低于对照组,差异有统计学意义(P<0.05);观察组总依从率高于对照组,差异有统计学意义(P=0.015);观察组总知晓率高于对照组,差异有统计学意义(P=0.025);观察组不良反应发生率低于对照组,差异有统计学意义(P=0.030)。结论:临床药师主导的远程慢性病药学服务可降低老年高血压患者的血压水平,提高服药依从性及药物知晓程度,减少不良反应。

Objective:To explore the application effect of clinical pharmacist-led remote pharmaceutical care for chronic diseases in elderly patients with hypertension.Methods:A total of two hundred and ten elderly patients with hypertension admitted to the Affiliated Hospital of Xuzhou Medical University from January 2022 to March 2023 were selected as the study objects,and randomly divided into control group and observation group,with one hundred and five patients in each group.The control group received routine chronic disease management,and the observation group received clinical pharmacist-led remote pharmaceutical care for chronic diseases.Blood pressure level,medication compliance,drug awareness and incidence of adverse reactions were compared between the two groups.Results:After intervention,the systolic blood pressure and diastolic blood pressure of the two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);the total compliance rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.015);the total awareness rate of observation group was higher than that of control group,and the difference was statistically significant(P=0.025);the incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P=0.030).Conclusion:Clinical pharmacist-led remote pharmaceutical care for chronic diseases can reduce blood pressure in elderly patients with hypertension,improve medication compliance and drug awareness,and reduce adverse reactions.
