经皮辅助关节囊上方入路人工全髋关节置换术在股骨头坏死患者中的应用效果

Application Effect of Percutaneous Assisted Capsular Approach for Total Hip Arthroplasty in Patients with Femoral Head Necrosis

目的:探究经皮辅助关节囊上方入路(SuperPATH)人工全髋关节置换术在股骨头坏死患者中的应用价值。方法:选取2019年12月—2020年12月丽江市人民医院收治的股骨头坏死患者60例为研究对象,随机分为对照组和研究组,各30例。对照组采用常规人工全髋关节置换术,研究组采用SuperPATH人工全髋关节置换术。比较两组手术切口长度、术中出血量、住院时间、视觉模拟评分法(VAS)评分及Harris评分。结果:研究组手术切口长度、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术前,两组VAS评分比较,差异无统计学意义(P>0.05);术后,两组VAS评分低于术前,研究组低于对照组,差异有统计学意义(P<0.05)。术前,两组Harris评分比较,差异无统计学意义(P>0.05);术后,两组Harris评分高于术前,研究组高于对照组,差异有统计学意义(P<0.05)。结论:SuperPATH人工全髋关节置换术在股骨头坏死患者中的应用效果较好,手术切口较小,术中出血量较少,可缓解术后疼痛,缩短住院时间,改善髋关节功能。

Objective:To explore the application value of percutaneous assisted capsular approach for total hip arthroplasty(SuperPATH)in patients with femoral head necrosis.Methods:Sixty patients with femoral head necrosis treated in Lijiang People's Hospital from December 2019 to December 2020 were selected as the study objects,and randomly divided into control group and study group,with thirty patients in each group.The control group underwent conventional total hip arthroplasty and the study group underwent SuperPATH total hip arthroplasty.Incision length,intraoperative blood loss,length of hospital stay,Visual Analogue Scale(VAS)and Harris score were compared between the two groups.Results:The length of surgical incision and hospital stay in the study group were shorter than those in the control group,and the amount of intraoperative blood loss was less than that in the control group,the difference was statistically significant(P<0.05).Before surgery,there was no significant difference in VAS scores between the two groups(P>0.05);after operation,VAS score of the two groups was lower than that before operation,and that of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).Before operation,there was no significant difference in Harris score between the two groups(P>0.05);after operation,the Harris score of the two groups was higher than that before operation,and the study group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:SuperPATH artificial total hip arthroplasty in patients with femoral head necrosis has a good effect,smaller incision,less intraoperative blood loss,can relieve postoperative pain,shorten hospital stay,and improve hip joint function.
