银杏叶提取物治疗特发性突聋患者的效果及对血清NLRP3、Caspase-1的影响

Effect of Ginkgo Biloba Extract on Patients with Idiopathic Sudden Deafness and Its Influence on Serum NLRP3 and Caspase-1

摘要 目的 探究银杏叶提取物治疗特发性突聋患者的效果及对血清Nod样受体蛋白3(NLRP3)、半胱氨酸蛋白酶-1(Caspase-1)的影响。方法 根据电子或纸质版的患者病历资料、相关检查或诊疗资料,收集2021年10月~2023年10月期间某院收治的80例特发性突聋患者临床相关资料,并依据治疗方案差异分组,联合组42例、对照组38例。对照组行常规治疗,联合组在对照组基础上联合使用银杏叶提取物治疗,两组均连续治疗10 d。比较两组的疗效、不良反应;比较两组治疗前后的炎症指标、抗凝血蛋白指标以及血清NLRP3、Caspase-1 m RNA水平变化。结果 联合组治疗总有效率较对照组显著更高(P<0.05);治疗后两组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)水平以及血清NLRP3、Caspase-1 m RNA表达较治疗前均下降,抗凝因子蛋白C(PC)、抗凝因子蛋白S(PS)、抗凝血酶Ⅲ(AT-Ⅲ)活性较治疗前均升高,且联合组改善优于对照组(均P<0.05);治疗后两组的各频率平均听阈较治疗前均改善,且联合组改善优于对照组(P<0.05);两组治疗期间不良反应情况无明显差异(P>0.05)。结论 银杏叶提取物治疗对特发性突聋患者的疗效显著,可改善患者炎症因子及凝血蛋白水平,促进听力功能恢复,其可能与下调NLRP3、Caspase-1表达有关。 OBJECTIVE To investigate the effect of ginkgo biloba extract on patients with idiopathic sudden deafness and its impact on serum levels of NOD-like receptor protein 3(NLRP3)and Caspase-1.METHODS SClinical data of 80 patients with idiopathic sudden deafness admitted from October 2021 to October 2023 were collected.Patients were divided into two groups based on treatment plans:a combination group of 42 patients and a control group of 38 patients.The control group received conventional treatment,while the combined group received the same treatment plus ginkgo biloba extract,with both groups treated for 10 consecutive days.The efficacy and adverse reactions of the two groups were compared.As well as the inflammatory indexes,anticoagulant protein indexes and serum NLRP3 and Caspase-1 mRNA levels before and after treatment.RESULTS The total effectiveness rate of the combination group was significantly higher than that of the control group(P<0.05).After treatment,the levels of IL-6,TNF-α,CRP,and the mRNA expressions of serum NLRP3 and Caspase-1,were decreased in both groups.Activities of anticoagulant proteins PC,PS,and AT-III increased compared to pre-treatment levels,with the combination group showing superior results(P<0.05).Average hearing threshold at all frequenciesimproved post-treatment,with the combination group showing greater improvements(P<0.05).There was no significant difference in adverse reactions between the groups(P>0.05).CONCLUSION Ginkgo biloba extract significantly improves outcomes for patients with idiopathic sudden deafness,potentially by downregulating the expression of NLRP3 and Caspase-1,and can improve inflammatory factors and coagulation protein levels,promoting recovery of hearing function.
