基于Miller金字塔原理的模拟教学法对内科住院医师及实习医生的影响

Observation on the Application of Simulation Teaching Under Miller Pyramid Principle in the Teaching of Internal Medicine Residents and Interns

摘要 目的探讨在内科住院医师及实习医生中采用基于Miller金字塔原理的模拟教学法的应用效果。方法选取2021年1月-2023年7月深圳恒生医院内科的住院医师及实习医生共56名作为研究对象,采用随机信封法分为A组(28名)与B组(28名),A组给予常规教学法,B组在A组的基础上给予基于Miller金字塔原理的模拟教学法,比较2组考核成绩(理论考试、实践考试)、岗位胜任能力及教学满意度。结果教学后,A组出科理论考试成绩为(81.37±3.15)分,实操考试成绩为(80.63±3.96)分,B组出科理论考试成绩为(91.43±3.81)分、实操考试成绩为(90.49±4.32)分,2组差异有统计学意义(P<0.05);B组岗位胜任能力各维度(临床基本能力、医生职业素质、掌握运用医学知识、医患沟通能力、团队合作能力、基本公共卫生合作能力、信息与管理能力、学术研究能力)评分较A组均更高,差异有统计学意义(P<0.05);2组教学满意度比较,B组教学满意度(92.86%)高于A组(71.43%),差异有统计学意义(P<0.05)。结论对内科住院医师及实习医生采用基于Miller金字塔原理的模拟教学法,能够从理论与实操层面提升医师专业能力与岗位胜任能力,因此其对该教学模式满意程度较高。 Objective To explore the application effect of Miller pyramid principle in simulated teaching among internal medicine residents and interns.Methods A total of 56 residents and interns from in the department of internal medicine of Shenzhen Hengsheng Hospital from January 2021 to July 2023 were selected as the research subjects.They were randomly divided into group A(28 resident)and group B(28 resident)using the envelope method.Group A received routine teaching method,while group B received Miller’s pyramid principle simulation teaching method based on group A.The assessment scores(theoretical and practical exams),job competence,and teaching satisfaction of the two groups were compared.Results After teaching,group A scored(81.37±3.15)in the theoretical exam and(80.63±3.96)in the practical exam,while group B scored(91.43±3.81)in the theoretical exam and(90.49±4.32)in the practical exam.The difference between the two groups was statistically significant(P<0.05);The scores of various dimensions of job competency in group B(clinical basic ability,doctor professional quality,mastery and application of medical knowledge,doctor-patient communication ability,teamwork ability,basic public health cooperation ability,information and management ability,academic research ability)were higher than those in group A,and the difference was statistically significant(P<0.05).The comparison of teaching satisfaction between two groups showed that the satisfaction rate of group B(92.86%)was higher than that of group A(71.43%),and the difference was statistically significant(P<0.05).Conclusion The Miller pyramid principle simulation teaching method for internal medicine residents and interns can improve physicians’professional ability and job competence from the theoretical and practical levels,so their satisfaction level with this teaching model is high.
