白头翁汤联合美沙拉嗪治疗溃疡性结肠炎的临床疗效

Clinical efficacy of Pulsatilla decoction combined with mesalazine in the treatment of ulcerative colitis

目的探析白头翁汤联合美沙拉嗪对溃疡性结肠炎的临床疗效。方法选取2020年5月至2022年4月龙口市中医医院收治的88例溃疡性结肠炎患者作为研究对象,采用随机数字表法分为对照组与观察组,每组44例。对照组采用美沙拉嗪口服联合安慰剂灌肠治疗,观察组采用白头翁汤(随证加减)浓缩液灌肠联合美沙拉嗪口服治疗。比较两组临床疗效、治疗前后炎症因子[白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)、C反应蛋白(CRP)、单核细胞趋化因子-1(MCP-1)]水平及症状(黏液脓血便、里急后重、腹痛、肛门灼热)积分。结果观察组治疗总有效率为93.18%,高于对照组的77.27%,差异有统计学意义(P<0.05)。治疗2个疗程后,两组IL-1β、IL-8、CRP、MCP-1水平均低于治疗前1 d,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗2个疗程后,两组黏液脓血便、里急后重、腹痛、肛门灼热积分均低于治疗前1 d,且观察组低于对照组,差异有统计学意义(P<0.05)。结论白头翁汤灌肠联合美沙拉嗪口服治疗溃疡性结肠炎效果显著,可有效抑制其胃肠道炎症反应,缓解临床症状,达到病情控制效果。

Objective To explore the clinical efficacy of Pulsatilla decoction combined with mesalazine in the treatment of ulcerative colitis.Methods 88 patients with ulcerative colitis who were treated in Longkou Traditional Chinese Medicine Hospital from May 2020 to April 2022 were selected as the research subjects,and they were divided into the control group and the observation group according to the random number table method,with 44 cases in each group.The control group was treated with oral mesalazine combined with placebo enema,while the observation group was treated with concentrated solution enema of Pulsatilla decoction(plus or minus with syndrome)combined with oral mesalazine.The clinical efficacy,levels of inflammatory factors(interleukin-1β[IL-1β],interleukin-8[IL-8],C-reactive protein[CRP],monocyte chemoattractant factor-1[MCP-1])and symptom(mucus purulent stool,tenesmus,abdominal pain,anal burning)score before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 93.18%,which was higher than that 77.27%in the control group,and the difference was statistically significant(P<0.05).After 2 courses of continuous treatment,the levels of IL-1β,IL-8,CRP,and MCP-1 of the two groups were lower than those 1 d before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).After 2 courses of continuous treatment,the scores of mucus bloody stool,tenesmus,abdominal pain and anal burning heat of the two groups were lower than those 1 d before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).Conclusion Pulsatilla decoction combined with mesalazine oral treatment of ulcerative colitis is effective,can effectively inhibit the gastrointestinal inflammatory response,alleviate clinical symptoms,and achieve the effect of disease control.
