固元扶阳灸联合针刺治疗寒湿阻滞型腰痛的临床观察

Clinical observation of Guyuan Fuyang moxibustion combined with acupuncture in the treatment of colddamp blockage type lumbago

目的探讨固元扶阳灸联合针刺治疗寒湿阻滞型腰痛的临床疗效。方法选取2021年4月至2022年4月江苏省第二中医院针灸科收治的48例寒湿阻滞型腰痛患者作为研究对象,按照奇偶法分为对照组与观察组,每组24例。对照组采用常规针刺治疗,观察组采用固元扶阳灸联合常规针刺治疗。比较两组临床疗效、视觉模拟评分法(VAS)评分、日本骨科协会(JOA)评估治疗评分。结果观察组治疗总有效率为91.7%,高于对照组的66.7%,差异有统计学意义(P<0.05)。治疗7、14 d后,两组VAS评分均低于前一时间点,差异有统计学意义(P<0.05);治疗14 d后,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);治疗前、治疗7 d后,两组VAS评分比较差异无统计学意义。治疗7、14 d后,两组主观症状、临床体征、日常活动受限度评分均高于前一时间点,差异有统计学意义(P<0.05);治疗14 d后,观察组主观症状治、日常活动受限度评分均高于对照组,差异有统计学意义(P<0.05);两组其他时间点主观症状、临床体征、日常活动受限度评分比较差异无统计学意义。结论固元扶阳灸联合针刺治疗寒湿阻滞型腰痛的临床疗效显著,可明显减轻患者疼痛,改善其临床表现,值得临床推广。

Objective To explore the clinical efficacy of Guyuan Fuyang moxibustion combined with conventional acupuncture in the treatment of cold-damp blockage type lumbago.Methods 48 patients with cold-damp blockage type lumbago admitted to the Acupuncture department of the Second Hospital of Traditional Chinese Medicine of Jiangsu Province from April 2021 to April 2022 were selected as the study subjects,and they were divided into the control group and the observation group according to the odd-even method,with 24 cases in each group.The control group was treated with conventional acupuncture,and the observation group was treated with Guyuan Fuyang moxibustion combined with conventional acupuncture.The clinical efficacy,visual analogue scale(VAS)score,and Japanese Orthopaedic Association(JOA)assessment treatment score were compared between the two groups.Results The total effective rate of treatment in the observation group was 91.7%,which was higher than 66.7%in the control group,and the difference was statistically significant(P<0.05).After 7 and 14 d of treatment,VAS scores of the two groups were lower than those of the previous time point,and the differences were statistically significant(P<0.05);after 14 d of treatment,the VAS score in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05);there were no significant differences in VAS scores between the two groups before and after 7 days of treatment.After 7 and 14 d of treatment,the subjective symptoms,clinical signs and daily activity tolerance scores of the two groups were higher than those of the previous time point,and the differences were statistically significant(P<0.05);after 14 d of treatment,the subjective symptom and daily activity tolerance scores in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05);there were no significant differences between the two groups in subjective symptoms,clinical signs and daily activities at other time points.Conclusion Guyuan Fuyang moxibustion combined with acupuncture has significant clinical efficacy in the treatment of cold-damp blockage type lumbago,which can significantly reduce the pain of patients and improve their clinical manifestations,which is worthy of clinical promotion.
