子宫静脉内平滑肌瘤超声检查特征及漏、误诊分析

Ultrasound characteristics and analysis of the missed and misdiagnosed uterine venous leiomyoma

目的 探讨子宫静脉内平滑肌瘤(IVL)超声特征及漏、误诊分析,以提高超声诊断准确率。方法 选取在我院检查并经手术病理确诊的IVL患者15例,分析超声声像图特征,探讨漏、误诊原因。结果 15例中超声诊断明确4例(准确率26%),误诊11例(误诊率74%)。其中误诊为子宫肌瘤9例(1例误诊为阔韧带肌瘤,1例误诊为子宫腺肌病);误诊为附件区肿瘤2例。结论 IVL缺乏特异性表现及超声科医师对于该病的认识有限,是导致漏、误诊的重要原因。

Objective To explore the ultrasound characteristics and analysis of the missed and misdiagnosed uterine venous leiomyoma,so as to improve the accuracy of ultrasound diagnosis.Methods Fifteen patients with uterine venous leiomyoma di-agnosed by pathological examination and surgical treatment in our hospital were selected to analyze the characteristics of ultra-sound images and to explore the reasons for missed and misdiagnosis.Results Among the 15 IVL patients,4 cases(26%)were clearly diagnosed by ultrasound,and 11 cases(74%)were misdiagnosed.Among them,9 cases were misdiagnosed as uterine fibroids(1 case was misdiagnosed as broad ligament fibroids,and 1 case was misdiagnosed as adenomyosis);Two cases were misdiagnosed as tumors in the accessory area.Conclusion The lack of specific manifestations in IVL and the limited under-standing and alertness of ultrasound physicians towards this disease are important reasons for the missed and misdiagnosis of this disorder.
