类风湿关节炎患者血清镍纹蛋白和疾病活动度及血脂谱的相关性

Association of serum meteorin-like protein with the disease activity and blood lipid profile of rheumatoid arthritis

目的探讨类风湿关节炎(RA)患者血清镍纹蛋白(Metrnl)和疾病活动度及部分脂质指标的相关性。方法66例RA患者根据28个关节炎疾病活动性评分(DSA28)分为低中度组(n=33)和重度组(n=33),比较2组RA患者的一般资料、实验室指标[类风湿因子(RF)、抗环瓜氨酸多肽抗体(anti-CCP)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、血细胞沉降率(ESR)、甘油三脂(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)]及血清Metrnl水平,采用Spearman分析不同疾病活动度RA患者IL-6、ESR、CRP、DSA28评分、RF及anti-CCP与血清Metrnl的相关性;根据是否合并高脂血症将RA患者分为单纯RA组(n=41)和RA合并高脂血症组(n=25),比较2组RA患者血清TG、TC、HDL-C、LDL-C及Metrnl水平,采用Spearman分析2组RA患者脂质指标与血清Metrnl的相关性。结果与低中度组RA患者比较,重度组RA患者的DSA28评分、IL-6、CRP、ESR及Metrnl水平升高(P<0.05),Spearman相关性分析表明不同疾病活动度RA患者的ESR、CRP、DSA28评分及RF与血清Metrnl呈正相关(r=0.3717、0.3009、0.8203、0.6650,P<0.05);与单纯RA组比较,RA合并高脂血症组患者的TG及血清Metrnl水平升高(P<0.05),TC与血清Metrnl呈负相关(r=-0.2453,P<0.05)。结论Metrnl与RA患者疾病活动度及血脂谱脂质指标变化有一定的相关性。

Objective To explore the association of serum Meteorin-like protein(Metrnl)with the disease activity and lipid profile of rheumatoid arthritis(RA).Methods Based on the disease activity score-28(DAS28),66 patients with RA were categorized into a low-to-moderate group(n=33)and a severe group(n=33).Two groups of RA patients were compared with general information and laboratory indexes[rheumatoid factor(RF),anti-cyclic citrullinated peptide(anti-CCP),interleukin-6(IL-6),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)],and serum Metrnl.Spearman correlation was employed to analyze the associations of IL-6,ESR,CRP,DSA28 score,RF,and anti-CCP with serum Metrnl in the patients with varying levels of disease activity.Based on whether hyperlipidemia was present or not,the patients with RA were classified into RA-only group(n=41)and RA accompanied by hyperlipidemia group(n=25).Two groups of the patients with RA were compared with the levels of serum TG,TC,HDL-C,LDL-C,and Metrnl.Spearman correlation was used to analyze the correlation between these lipid parameters and serum Metrnl.Results When compared to the patients with low-to-moderate RA,the patients with severe RA exhibited the elevated levels of DAS28 score,IL-6,CRP,ESR,and Metrnl(P<0.05).Spearman correlation analysis revealed the positive associations of ESR,CRP,DAS28 score,and RF with serum Metrnl in patients with RA across varying disease activity(r=0.3717,0.3009,0.8203,0.6650;P<0.05).When compared to the patients with RA-only,the patients with RA accompanied by hyperlipidemia had the increased levels of TG and serum Metrnl(P<0.05).TC was negatively correlated with serum Metrnl(r=-0.2453,P<0.05).Conclusion Metrnl has a certain correlation in predicting disease activity and lipid profile changes in patients with RA.
