激光切除术治疗声带白斑对患者嗓音功能恢复的临床疗效分析

Analysis on Effect of Laser Excision for Vocal Cord Leukoplakia on Recovery of Voice Functions

目的探讨激光切除术治疗声带白斑对患者嗓音功能恢复的效果。方法选择2019年10月至2024年1月,声带白斑患者60例,随机分为对照组和手术组,每组患者30例。其中,对照组予以传统剥脱术;手术组予以CO_(2)激光切除术,切割功率1~2 W、超脉冲模式、脉宽时间0.1 ms。术后2周,检测两组患者嗓音包括振幅微扰、基频微扰的变化情况;比较术后2周、术后1个月两组患者嗓音障碍指数(voice handicap index,VHI)情况;对手术疗效进行评估。结果术后2周两组患者嗓音变化指数较手术前升高,振幅微扰、基频微扰较手术前降低(P<0.05)。手术组术后2周振幅微扰、基频微扰低于传统剥脱术组(P<0.05)。术后2周、术后1个月两组患者主观VHI较手术前均降低(P<0.05),手术组VHI均低于对照组(P<0.05)。术后2周,手术组术后疗效优于对照组(P<0.05)。两组术后均未出现严重并发症。结论激光切除术治疗声带白斑对患者嗓音功能恢复起到积极改善的作用。

Objective To explore the effect of laser excision treatment of vocal cord leukoplakia on the recovery of patients'voice functions.Methods Totally 60 patients with vocal cord leukoplakia treated between Oct.2019 and Jan.2024 were selected and randomly divided into a control group and an observation group.While the patients in the control group being given traditional exfoliation surgery,the patients in the surgical group received the CO_(2)laser resection(in super pulse mode)with a cutting power of 1-2 W and a pulse width time of 0.1 ms.The changes to the patients’voice,including amplitude perturbations and fundamental frequency perturbations were analyzed 2 weeks after the surgery,the voice handicap index(VHI)and surgical efficacy 2 weeks and 1 month after the surgery were compared respectively.Results In both groups,the voice change index increased and the amplitude perturbation and fundamental frequency perturbation decreased after the surgery(P<0.05).Moreover,the amplitude perturbation and fundamental frequency perturbation in the observation group were lower than those in the traditional exfoliation group(P<0.05).The subjective VHI decreased after the surgery in both groups(P<0.05),and the VHI was lower in the surgical group than in the control group(P<0.05).The postoperative efficacy of the surgical group was better than that of the control group(P<0.05).No serious complication occurred in either group after the surgery.Conclusions Laser resection for vocal cord leukoplakia plays a positive role in improving the recovery of patients'voice functions.
