日间手术模式下耳穴压丸疗法对肛周脓肿术后疼痛临床观察

Clinical Observation on Auricular Point Pressure in Relieving Pain after Ambulatory Surgery for Perianal Abscesses

目的 观察耳穴压丸疗法对肛周脓肿术后患者疼痛的疗效。方法 选取2023年1月1日—2023年9月30日北京市肛肠医院(北京市二龙路医院)肛肠急诊科收治的86例肛周脓肿于日间手术模式下行切开引流术的患者为研究对象,随机分为对照组和治疗组,对照组:术后抗感染等基础治疗。治疗组:在对照组基础治疗上增加耳穴压丸治疗。术后6 h及第1、2、5天进行电话随访或来院进行术后复查换药,采用数字评分法评估患者疼痛情况,对比两组不同时间点疼痛程度[视觉模拟评分法(VAS)],并记录止痛药的使用情况。结果 两组患者术后6 h、术后第1天、术后第2天、术后第5天的VAS评分均低于术前,两组间术后6 h、术后第1天、术后第2天不同时间点的VAS评分,治疗组低于对照组,差异有统计学意义(P <0.05)。结论 日间手术模式下,耳穴压丸疗法能显著降低肛周脓肿患者术后的疼痛。

Objective To observe the therapeutic effect of auricular point pressure on the pain after perianal abscess surgery.Methods Eighty-six patients with perianal abscesses treated with open drainage in ambulatory surgery in the anorectal emergency department of Beijing Rectum Hospital(Beijing Er Long Lu Hospital)from January 1 to September 30 in 2023 were enrolled in this study and randomized into a control group and a treatment group.The control group received basic therapies such as postoperative anti-infection therapy.The treatment group received auricular point pressure in addition to the basic therapies in the control group.6 h and on days 1,2,5 after surgery,the patients were followed up by telephone or came back to the hospital for postoperative review and dressing change.The visual analog scale(VAS)scores at different time points were compared between the two groups,and the use of painkillers was recorded.Results The VAS scores at the time point of 6 h and on days 1,2,and 5 after surgery were lower than that before surgery in both groups.Moreover,the VAS scores of the treatment group were lower than those of the control group at the time point of 6 h and on days 1 and 2 after surgery(P<0.05).Conclusion Auricular point pressure can relieve postoperative pain in the patients undergoing ambulatory surgery for perianal abscesses.
