钢板螺钉内固定技术治疗四肢长管骨创伤骨折患者的效果及对VAS评分、不良事件发生情况的影响

Effect of plate and screw internal fixation in the treatment of patients with traumatic fracture of long bones of extremities and its influences on VAS score and incidence of adverse events

目的探讨钢板螺钉内固定技术治疗四肢长管骨创伤骨折患者的效果及对视觉模拟评分法(VAS)评分、不良事件发生情况的影响。方法选取2020年1月至2023年1月收治的80例四肢长管骨创伤骨折患者为研究对象,依据不同治疗方案将其分为对照组和观察组,各40例。对照组应用闭合复位交锁髓内钉内固定术治疗,观察组应用钢板螺钉内固定技术治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。观察组的手术时间、住院时间及骨折愈合时间均短于对照组(P<0.05)。治疗后,两组的Fugl-Meyer运动功能评定量表(FMA)评分均升高,VAS评分均降低,且观察组优于对照组(P<0.05)。治疗后,观察组的心理健康、躯体功能、角色状态及社会功能评分均高于对照组(P<0.05)。治疗后,两组的骨钙素(BGP)、Ⅰ型前胶原羧基端肽(PⅠCP)水平均升高,且观察组高于对照组(P<0.05)。观察组的不良事件总发生率低于对照组(P<0.05)。结论钢板螺钉内固定技术治疗四肢长管骨创伤患者的效果较好,其不仅可缩短手术时间,加快康复进程,改善肢体功能,还能降低疼痛程度,进一步提高生活质量,防范不良事件,值得推广与应用。

Objective To investigate the effect of plate and screw internal fixation in the treatment of patients with traumatic fracture of long bones of extremities and its influences on Visual Analogue Scale(VAS)score and incidence of adverse events.Methods A total of 80 patients with traumatic fracture of long bones of extremities admitted from January 2020 to January 2023 were selected as the research objects.According to different treatment schemes,the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated with closed reduction and interlocking intramedullary nail internal fixation,and the observation group was treated with plate and screw internal fixation.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).The operation time,hospitalization time and fracture healing time of the observation group were shorter than those of the control group(P<0.05).After treatment,the Fugl-Meyer Assessment(FMA)score of the two groups increased,the VAS score decreased,and those in the observation group were better than the control group(P<0.05).After treatment,the scores of mental health,physical function,role status and social function in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of bonegla protein(BGP)and procollagen typeⅠcarboxyl terminal peptide(PⅠCP)in the two groups increased,and those in the observation group were higher than the control group(P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group(P<0.05).Conclusion Plate and screw internal fixation in the treatment of patients with traumatic fracture of long bones of extremities has better effect,it can not only shorten the operation time,accelerate the rehabilitation process,improve limb function,but also reduce the degree of pain,further improve the quality of life and prevent adverse events,which is worth promoting and applicating.
